Consent for Initial Evaluation/Reevaluation

Set up Consent for Initial Evaluation/Reevaluation

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Consent for Initial Evaluation/Reevaluation under Part B: Referral, and click Go.

  4. In the Student Information section, the system auto-populates the following information:

    • From the student's Student Info/Demo Part B profile:

      • Student

      • USI

      • DOB

    • From the student's LEA Entry/Exit Part B profile:

      • Grade

    • From the student's Special Education Part B profile:

      • Case Manager

      • Last Eligibility Meeting Date

      • Last IEP Annual Review Meeting Date

  5. Select the parent/guardians who wish to sign the consent. The parents' or guardians' names appear from the Student Contacts Part B profile, where the Educational Rights checkbox is selected.

  6. The two checkboxes are only editable via digital signature. When the parent/guardian signs electronically and selects one of the two checkboxes, upon saving, the Consent response and Consent date will flow to the Part B Referrals student profile.

    • I give my consent to have <student name> evaluated to determine if he/she is eligible or continues to be eligible for special education and to determine educational needs. I understand this consent is voluntary and may be revoked at any time.

    • I DO NOT give my consent to have <student name> evaluated to determine if he/she is eligible for special education and to determine educational needs.

  7. When the parent/guardian signs on paper, select Parent will sign a paper copy checkbox. When the box is selected, the Document Delivery flyout section is required. The case manager will complete the Document Delivery section to indicate the Consent response and Consent date.

  8. In the Signature of Parent/Guardian area, a parent or guardian signature will appear through the digital signature functionality unless the user selects Parent will sign a paper copy checkbox.

  9. The signature date will auto-populate when the parent/guardian signs digitally.

  10. Select the checkboxes associated with each document the user wants to enclose. Here are the available documents:

    • Procedural Safeguards

    • Prior Written Notice

    • Analysis of Existing Data Report

  11. The system auto-populates the name of the school the student attends from the student's Profile, LEA Entry/Exit profile.

  12. The system provides optional textboxes to enter additional records during the evaluation process.

  13. Click Save, Done Editing.

  14. Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

  • If more than one parent has educational rights, the system provides a list of the parent or guardian names with a checkbox, and you must select the name of the parent or guardian who will sign digitally. The selected parents or guardians will only be displayed in print mode.

  • Click More, Update Document from Student Profile to refresh parent or guardian details on the document when the document in draft mode. Parent or guardian details cannot be updated for the document in Review or Final mode.

Document Delivery

This section is required when the user selects Parent will sign a paper copy checkbox. The section does not print.

  1. Within the Consent for Initial Evaluation/Reevaluation document, navigate to Document Delivery using the flyout menu.

  2. Select the Mode of Delivery from the drop-down list.

  3. Select the Date of Delivery.

  4. Select a response for Parent Consent from the drop-down list.

  5. Select the Parent Consent Date.

  6. Click Save, Done Editing.

  7. Finalize the document when completed.

  8. Upon finalization of the document, the Consent Response value and Parent Consent Date value will flow to the Referrals Part B profile.

    • At least three attempts to contact the parent must be made in the contact log before Did Not Respond can be selected in Document Delivery. If document delivery uses the date the LEA receives, the document will flow to the student's Profile, Referrals Part B, for example, Consent Received Date.
    • When the document is finalized with Parent Consent response as Did Not Respond, the system updates the eligibility status in the student's Profile, Referrals Part B.
    • Once the Referral profile is updated with Did Not Respond, the Special Education Part B, Eligibility Outcome appears as Discontinuation of Referral.
    • Once the Referral profile is updated with Consent Denied, the Special Education Part B, Eligibility Outcome appears as Parent denied consent.
