Consent for Initial Provision of Services
Consent for Initial Provision of Special Education and Related Services
Select a student on the Students panel.
Click the Documents icon for the student.
From the Create New Document drop-down list, select Consent for Initial Provision of Services under Individualized Education Plan, and click Go.
Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.
In the Student Information section, the system auto-populates the following information:
From the Student Info/Demo Part B profile:
Student
State USI
DOB
From the LEA Entry/Exit Part B profile:
Grade
From the Special Education Part B profile:
Case Manager
Last Eligibility Meeting Date
Last IEP Annual Review Meeting Date
The system displays the student's name as Dear Parent/Guardian of <student name> within the letter when the Student Holds Educational Rights checkbox is not selected within the Student Info/Demo Part B profile. Otherwise, it displays the student's name as Dear <Student Name>.
The system auto-populates the LEA name from the LEA Entry/Exit Part B profile.
Select the Parent/Guardian who wishes to sign the consent. The parents' or guardians' names appear from the Student Contacts Part B profile.
The two checkboxes are only editable through digital signature. When the parent or guardian signs digitally and selects one of the two checkboxes, the consent response and date will flow to the Referrals Part B profile upon saving.

When the user selects Parent/Guardian did not respond to request for consent for the initial provision of services checkbox, a note appears: An LEA must make reasonable efforts to obtain informed consent from the parent/guardian for the initial provision of special education and related services to the child.
In the Signature of Parent/Guardian area, a parent or guardian signature will appear through the digital signature functionality unless the user selects Parent/Guardian will sign a paper copy checkbox.
The signature date will auto-populate when the parent or guardian signs digitally.
When the parent or guardian signs on paper, select the Parent/Guardian will sign a paper copy checkbox. When this checkbox is selected, the Document Delivery flyout section is required. The case manager will complete the Document Delivery section to indicate the consent response and date.
Click Save, Done Editing.
Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.
If more than one parent has educational rights, the system provides a list of the parent or guardian names with a checkbox, and the user must select the name of the parent or guardian who will sign digitally. The selected parents or guardians will only be displayed in print mode.
Document Delivery
This section is only required when the user selects Parent/Guardian will sign a paper copy checkbox. This section does not print.
Within the Consent for Services document, navigate to Document Delivery using the flyout menu.
Select a Mode of Delivery.
Enter a date manually or select a date using the calendar icon in the Date of Delivery field.
Select a Parent Consent. This field value will flow to the Referrals Part B profile, Consent Response for Initial Provision of Services field upon finalization of the document.
Enter a date manually or select a date using the calendar icon in the Parent Consent Date field. This field value will flow to the Referrals Part B profile, Consent Date for Initial Provision of Services field upon finalization of the document.
Click Save, Done Editing.
