Evaluation Report
Evaluation Report is a standard Medicaid report that captures data from the student's Assessments Part B profile available in the PowerSchool Special Programs state model for the District of Columbia. The report displays all the assessments completed within the past year from the report generation date.
Submission Requirements
This report includes students who have completed their assessment within the last 365 days and the assessment category falls under Adaptive Behavior, Auditory Processing, Developmental, Neuropsychological, Occupational Therapy, Orientation and Mobility, Physical Therapy, Physical Therapy Screening, Psychiatric, Psychological, Speech Language, or Speech Language Screening.
Generate Report
From the Reporting menu, choose Standard Reports.
Click Part B Medicaid, then click Evaluation Report.
To review the latest data, click Refresh after opening the report in the application.
