Independent Student Drop-Off Consent Form

This page will be automatically added as a section of the IEP when the student is 12 years of age or older as of the IEP meeting date and where the student is eligible for specialized transportation, ASP, as indicated in the Services and Dedicated Aides section.

Independent Student Drop-Off Consent Form

  1. Within the IEP document, navigate to Independent Student Drop-Off Consent Form using the flyout menu.

  2. In the Student Information section, the system auto-populates the following information:

    • From the Student Info/Demo Part B profile:

      • Student

      • State USI

      • DOB

    • From the LEA Entry/Exit Part B profile:

      • Grade

    • From the Special Education Part B profile:

      • Case Manager

      • Last Eligibility Meeting Date

      • Last IEP Annual Review Meeting Date

  3. Each Parent/Guardian name will appear where the Educational Rights checkbox is selected in the Student Contacts Part B profile. Select each parent or guardian’s name as desired.

  4. For the Signature of Parent/Guardian, the system is configured for the digital signature, or the Parent/Guardian can sign on a paper copy.

  5. For Date, the system auto-populates the date of the digital signature, or the Parent/Guardian can sign on a paper copy.

  6. Click Save, Done Editing.

  • The system auto-populates the student's full name and the parent's or guardian's name in the on-screen message where the parent or guardian gives consent to the Office of the State Superintendent of Education Division of Student Transportation (OSSE-DOT) to drop the student at the address listed on the Transportation Request Form.

  • Once the IEP is finalized, this section is editable in final. The user can edit the section and add the Date Received, School/Staff Contact (Title will auto-populate if the Staff lookup is used. The form will auto-populate the LEA Name and School Name from the LEA Entry/Withdrawal profile.

  • The header of the page displays the Office of the State Superintendent of Education (OSSE) and its address.
