Initial Eligibility List

This report shows details for students whose initial eligibility evaluation is due. This report helps schools or Local Educational Agencies (LEAs) track the student's expected initial eligibility schedule when referred for initial evaluation, transitioning from Part C to B, or having an out-of-state Individualized Education Program (IEP).

Submission Requirements

The report lists all the students with active enrollment and not exited the special education program and are yet to be evaluated for their eligibility for special education service. The report calculates the Initial Eligibility Due Date based on the reason for referral and the date of consent for an initial evaluation.

School-specific reports can be generated using the Organization Location Filter.

Students with a referral for an initial evaluation or transitioning from Part C to Part B should be evaluated within 60 days since the parent or guardian has provided the consent for evaluation.

For students with an IEP from other jurisdictions and are referred to a special program with an out-of-state IEP, the initial evaluation will be considered timely if it takes place within ten days from the receipt of consent for evaluation.

Generate Report

From the Reporting menu, choose Standard Reports.
Click Part B Model Reports, then click Initial Eligibility List.

To review the latest data, click Refresh after opening the report in the application.
Select a school from the Organization Location Filter drop-down menu to generate a school-specific report.
