Notice and Signature
Within the Service Plan document, navigate to Notice and Signature using the flyout menu.
The system auto-populates:
Student Name, USI, and DOB from the Student Info/Demo Part B profile.
Grade from the LEA Entry/Exit Part B profile.
The system provides the following four checkboxes under the Parent/Guardian/Student Acknowledgement and Commitment area:
A copy of the Notice of Procedural Safeguards has been given to Parent/Guardian/Student: The user must select this checkbox when the parent, guardian, or student signs digitally. This checkbox is editable only via digital signature, not on UI. However, it is displayed on UI all the time.
I, as Parent/Guardian/Student, understand the Service Plan content and provision and I have been fully informed of my procedural rights and (select one): This checkbox is required when a parent, guardian, or student signs digitally.
I accept Special Education services on the Service Plan.
I decline Special Education services outlines on this Service Plan at this time.
When the parent, guardian, or student signs on paper, select the Parent/Guardian/Student will sign a paper copy checkbox. When the checkbox is selected, the Document Delivery section is then required.
The system auto-populates the parent or guardian names with Educational Rights selected in the Student Contacts Part B profile. The user can choose the parent or guardian who wishes to sign.
The system provides a series of checkboxes to confirm the parent or guardian's responses. The user must select at least one checkbox.
In the Parent/Guardian Signature or Student Signature area, a parent, guardian, or student's signature will appear through the digital signature functionality unless the user selects Parent/Guardian/Student will sign a paper copy checkbox.
In the Date area, the signature date will auto-populate when the parent, guardian, or student signs digitally.
Click Save, Done Editing.
Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.
If the parent, guardian, or student signs digitally in the Parent/Guardian/Student Acknowledgement and Commitment area, they must select I accept or I decline checkbox. If the parent, guardian, or student signs on a paper copy, then I accept or I decline must be noted in the Document Delivery section before the document is finalized.
Document Delivery
This section is only required when the user selects Parent/Guardian/Student will sign a paper copy checkbox. The section does not print.
Within the Service Plan document, navigate to Document Delivery using the flyout menu.
The system auto-populates:
Student Name, USI, and DOB from the Student Info/Demo Part B profile.
Grade from the LEA Entry/Exit Part B profile.
Select the Mode of Delivery.
Enter a date manually or select a date using the calendar icon in the Date of Delivery field.
Select a Parent/Guardian/Student Response. If the Response is Parent Rejects/Does Not Approve or Parent Requests Meeting, the document will not display as Active, and no data will flow to any student profiles.
Enter a date manually or select a date using the calendar icon in the Parent/Guardian/Student Response Date field.
Click Save, Done Editing.
The Delivery Date and Parent/Guardian/Student Response Date must be the same as or later than the Service Plan Meeting Date on the Cover Sheet page.
Data Flow
Upon completing the document, the following data flow occurs:

Approved Service Plan
If the Response is Parent Rejects/Does Not Approve or Parent Requests Meeting, the document will not display as Active, and no data will flow to any student profiles.
Enter a date manually or select a date using the calendar icon in the Parent/Guardian/Student Response Date field.
Upon finalization, if the Parent Response is either Parent Rejects/ Does Not Approve or Parent Requests Meeting, the Service Plan document will not appear as ACTIVE, and the data will not flow from the Service Plan document to the student’s profiles.
