Parent Input Form

This form includes questions about the student's educational history, medical information, and any previous evaluations or assessments that have been conducted. The form also asks for information about the student's strengths and weaknesses and any goals or concerns the parents or guardians have for their student's education.

Parent Input Form

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Parent Input Form under Individualized Education Plan, and click Go.

  4. Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.

  5. In the Student Information section, the system auto-populates the following information:

    • From the student's Student Info/Demo Part B profile: 

      • Student

      • USI

      • DOB

    • From the student's LEA Entry/Exit Part B profile:

      • Grade

    • From the student's Special Education Part B profile:

      • Case Manager

      • Last Eligibility Meeting Date

      • Last IEP Annual Review Meeting Date

  6. Enter a date manually or select a date using the calendar icon.

  7. The system auto-populates the parent's or guardian's name and address that has Educational Rights selected on the Student Contacts Part B profile. Select at least one parent or guardian. When the user selects the Red Flag checkbox in the Student Info/Demo Part B profile, the parent's or guardian's address will be suppressed.

  8. The system displays the student's name as Dear Parent/Guardian of <student name> within the letter, asking parents or guardians to meet to discuss the student's IEP. Users must enter a date manually or select a date using the calendar icon to confirm when they wish to return this form to the school the student attends. The system generates separate letters for each parent or guardian when the user selects more than one parent or guardian.

  9. The system provides the following long stylized text boxes to type the student's educational history, medical information, and any previous evaluations or assessments:

    • What are the strengths of your child?

    • What motivates your child?

    • Are there areas of concern regarding your child that we should be aware of:

      • when he/she is at home?

      • when he/she is at school?

    • What techniques, if any, have you used to address the concern(s) noted above? Were they successful?

    • Does your child have any medical concerns that you want to share?

    • What is/are the most important goal(s) that you would like to see accomplished in the upcoming year?

    • Is there any other information that we should know that would assist us in developing the IEP?

    • Do you have any questions for the IEP team or about the IEP process?

  10. When the user selects the Parent/Guardian will receive a paper copy checkbox, the system adds a Document Delivery section to this document and the user must complete this section before finalizing the form. The checkbox and text will be suppressed during printing or in view mode.

  11. Click Save, Done Editing.

  12. Finalize the document when completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

  • The user must type a minimum of 20 characters in each text box.

  • If the document is given to parents or guardians in paper form, the system will provide space between the questions to write the answers.

Document Delivery

This section is only required when the user selects the Parent/Guardian will receive a paper copy checkbox. The section does not print.

  1. Within the Parent Input Form document, navigate to Document Delivery using the flyout menu.

  2. Select a Mode of Delivery.

  3. Enter a date manually or select a date using the calendar icon in the Date of Delivery field.

  4. Click Save, Done Editing.
