Plan Dates
Plan Date is a standard report that holds the student’s current special education plan details like an Individualized Education Program (IEP), Service Plan(SP), 504 Plan and Gifted IEP, and so on. The report shows the plan durations in terms of the start and end date of the existing plan, that is Annual Review date.
Submission Requirements
The report lists all students with an active special education plan, such as IEP, Service Plan, 504 plan, or Gifted IEP plan. The report lists only students who are active in special education.
Generate Report
From the Reporting menu, choose Standard Reports.
Click Part B Medicaid, then click Plan Dates.
To review the latest data, click Refresh after opening the report in the application.
Plan Start Date, Plan End Date, Plan ID, and Plan Type columns are repeated in the IEP, Service Plan, and Gifted IEP report. The given Plan Type column will show the Plan the student currently holds.
