Reasonable Efforts To Obtain Parental Consent

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Reasonable Efforts To Obtain Parental Consent under Part B: Referral, and click Go.

  4. In the Student Information section, the system auto-populates the following information:

    • From the Student Info/Demo Part B profile:

      • Student

      • USI

      • DOB

    • From the LEA Entry/Exit Part B profile:

      • Grade

    • From the Special Education Part B profile:

      • Case Manager

      • Last Eligibility Meeting Date

  5. The system auto-populates the Date of referral from the student's latest Referrals Part B profile.

  6. The system calculates the First contact due date, which is the referral date plus ten business days (in-session days).

  7. The system calculates the date for the Complete reasonable efforts field, which is the referral date plus 30 calendar days.

  8. The system auto-populates the Date of parental consent for initial evaluation from the student's latest Referrals Part B profile.

  9. The system auto-selects a response for the Consent Response checkbox based on the selected response in the student's Referrals Part B profile.

  10. Select a parent or guardian to add contact attempt details. The parent or adult student names appear from the student's Student Contacts Part B profile, where the Educational Rights checkbox is selected.

    • After selecting a parent or guardian, select a value for each of these fields:

      • Contact Date

      • Contact Type (If selecting Other, a textbox appears to describe the type.)

      • Contact Reason (If selecting Other, a textbox appears to describe the reason.)

      • Contact Method (User must select two different methods for the three contact dates. If selecting Other, a textbox appears to describe the method.)

      • Result/Notes

  11. The system selects Yes as a default response for Were reasonable efforts completed within 30 days of referral? checkbox during the following scenarios:

    • If there is a maximum of three contact dates on different dates with two different methods, regardless of the result, all dates are five days less than the date mentioned in the Complete reasonable efforts field.

    • If there is a contact date within 30 days from the First contact due date less than the date mentioned in the Complete reasonable efforts field, the Result/Notes is selected either Parent denied consent for initial evaluation or Parent provided consent for initial evaluation.

  12. The system calculates and selects a response for the Was parental consent completed within 30 days of the referral? checkbox.

    • The system selects Yes when the Date of parental consent for initial evaluation is lesser than 30 calendar days from the Date of referral.

    • The system selects No when the Date of parental consent for initial evaluation is greater than 30 calendar days from the Date of referral.

  13. Enter a date for the If parent requested delay in the process, indicate the date to resume the initial evaluation process field.

  14. Click Save, Done Editing.

  15. Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

  16. When the document is finalized, the document will update the student's Referrals Part B profile: Were Reasonable Efforts Completed within 30 days, Was Parental Consent Obtained within 30 days and Parent requested delay, date to resume process values.

  • Users can submit up to three different contact dates.

  • The If parent requested delay in the process, indicate the date to resume the initial evaluation process field is required when selecting Parent requested delay/meeting rescheduled as a value in Result/Notes.
