Secondary Transition

This section of the IEP is required when the student is 13 years of age or older. The section is automatically inserted into the IEP if the student is age 13 or older using the IEP meeting date compared to the student's date of birth. At any time, the user can add this section for students under 13. To add, the user must click Set Document, Sections, select the Secondary Transition checkbox, and click Accept.

Set up Secondary Transition

  1. Within the IEP document, navigate to Secondary Transition using the flyout menu.

  2. The system auto-populates:

    • Student Name and USI from the Student Info/Demo Part B profile.

    • Meeting Date from the IEP cover sheet.

  3. The system displays the details of what the student will do after high school based on the transition assessment data. The texts can be edited.

  4. Select a response for the Did the student participate in the IEP Meeting? checkbox.

    • If the user selects No, a required text box appears.

  5. Select the applicable Assessment Type checkboxes and enter an end date for that assessment in the Date Completed field.

    • If the user selects any one of the following checkboxes, describe the assessment type:

      • Vocational assessment(s): Assessment Name

      • Other

  6. In the Attach Transition Assessment section, upload multiple files using the Attach File option.

  7. The system provides long stylized text boxes for the following fields:

    • Needs

    • Strengths

    • Preferences

    • Interests

    • Experiences

  8. In the Education/Training section, the system provides long stylized text boxes for the following fields:

    • Measurable Post-Secondary Goal(s)

    • Annual Transition Goals

  9. Select a response for the Is Rehabilitation Services Administration (RSA) or any other outside agency expected to be responsible for providing any of the transition services listed below? checkbox.

    • If the user selects Yes, a message appears saying Yes is selected thus the Participant Consent Letter is required.

  10. In the Transition Service section, the system provides a long stylized text box to type the transition services related to education or training and then select an appropriate checkbox for Staff/Agency Responsible.

    • If the user selects Other, they must describe the details of the staff or agency responsible.

  11. Enter the following details:

    • Projected Begin Date: The date must not extend one year minus one day from the IEP meeting date.

    • Projected End Date: The date must not extend one year minus one day from the IEP date.

    • Frequency: Select the appropriate frequency.

    • Duration: Select the number of Hours and Minutes.

    • Settings: Select the applicable settings.

  12. The Employment and Independent Living Skills sections provide the same fields as Education/Training section.

  13. In the Course of Study section, the system provides a long stylized text box to enter the course(s) of study needed to assist the student in reaching their post-secondary goals. The user can attach the list of courses using the Attach File option.

  14. Select the Anticipated Graduation Year.

  15. Select the Diploma Type.

  16. Click Save, Done Editing.

  • The user must type a minimum of 20 characters in each text box.

  • To add additional goals, click the arrow pointing to the right.
