Section 504 - Plan
This page outlines the accommodations and services that a student with a disability requires to access and participate in their education on an equal basis with their peers.
Section 504 Plan
Select a student on the Students panel.
Click the Documents icon for the student.
From the Create New Document drop-down list, select Section 504 - Plan under Section 504, and click Go.
Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.
In the Student Information section, the system auto-populates the following information:
From the Student Info/Demo Part B profile:
Student
USI
DOB
From the LEA Entry/Exit Part B profile:
Grade
From the Special Education Part B profile:
Last IEP Annual Review Meeting Date
From the Section 504 profile:
Section 504 Coordinator
Enter a date manually or select a date using the calendar icon. The date in this field flows back to the Date of Section 504 Accommodation Plan field in the Section 504 profile when the document is finalized.
In the Evaluation Summary section:
The system auto-populates the Section 504 Eligibility Date from the Section 504 profile.
The system provides two long stylized long textboxes to enter the student's disability:
Physical or mental impairment
Major life activity substantially limited by the impairment
Select the type of accommodations needed for the student from the following categories listed in the Accommodations/Modifications Needed grid:
Presentation
Response
Timing and Scheduling
Setting: If you select the Other checkbox, you must describe the accommodations needed.
Other: If you select the Other checkbox, you must describe the accommodations needed.
In the State Assessment Accommodations section. select the following from the associated drop-down list:
Assessment
Subject: When the user selects ACCESS or Multi State Alternate Assessment (MSAA) as an assessment, there is nothing to choose in the Subject column.
Accommodations
In the Transportation section, select a response for the Student is eligible for Transportation checkbox.
The system auto-populates the parent or guardian names that have Educational Rights selected in the Student Contacts Part B profile. The user can choose the parent or guardian who wishes to sign.
The system provides a series of checkboxes to confirm the parent or guardian consent. The user must select at least one checkbox.
In the Parent/Guardian Signature area, a parent or guardian signature will appear through the digital signature functionality unless the user selects Parent/Guardian will sign a paper copy checkbox.
In the Date area, the signature date will auto-populate when the parent or guardian signs digitally.
When the parent or guardian signs on paper, select the Parent/Guardian will sign a paper copy checkbox. When the checkbox is selected, the Document Delivery section is then required.
Click Save, Done Editing.
Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.
Upon finalization of the document, data from the State Assessment Accommodations section flows back to the Assessment Accommodations profile.
To add State Assessment Accommodations, click the plus icon.
Document Delivery
This section is only required when the user selects Parent/Guardian will sign a paper copy checkbox. The section does not print.
Within the Section 504 - Plan document, navigate to Document Delivery using the flyout menu.
Select the Mode of Delivery.
Enter a date manually or select a date using the calendar icon in the Date of Delivery field.
Click Save, Done Editing.
