Summary of Performance

The purpose of this page is to summarize a student's educational achievement, functional performance, recommendations for future accommodations, and the services required to support their post-secondary goals.

Summary of Performance

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Summary of Performance under Individualized Education Plan, and click Go.

  4. Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.

  5. In the Student Information section, the system auto-populates the following information:

    • From the Student Info/Demo Part B profile: 

      • Student

      • USI

      • Local ID

      • DOB

    • From the LEA Entry/Exit Part B profile:

      • Grade

    • From the Special Education Part B profile:

      • Case Manager

      • Last Eligibility Meeting Date

      • Last IEP Annual Review Meeting Date

  6. Enter a date manually or select a date using the calendar icon.

  7. In the Background Information section, perform the following activities:

    • Select an Assessment Type. The system provides a drop-down list with the linked assessment categories from the most recent finalized evaluation summary report.

    • Select an Assessment Name

    • Enter a date manually or select a date using the calendar icon.

    • The user can upload multiple files related to the most recent assessments of the student’s disability using the Attach File option.

  8. In the Student Perspective section, the system provides long stylized text boxes to answer the following questions:

    • How does your disability affect your academic work and school activities (such as grades, relationships, assignments, projects, communication, time on tests, mobility, extra-curricular activities)?

    • What supports have been tried by teachers or by you to help succeed in school (aids, adaptive equipment, physical accommodations, other services)?

    • Which accommodations and supports worked best for you?

    • Which accommodations and supports have not worked for you?

    • What strengths and needs should people know about you as you enter the postsecondary education or work environment?

  9. In the Summary of Present Levels section, the system displays the following fields:

    • The system auto-populates the Goal Area value from the IEP, Present Levels of Performance And Goals page within the Area of Concern field. To add more areas of concern, click the plus icon.

    • The system provides three long stylized text boxes to enter descriptions of a student's academic achievement and functional performance and how the student's disability affects the student's progress in the general education curriculum and access to the general education curriculum.

  10. In the Team Recommendations section, perform the following activities: 

    • Select a Type of Goal. When the user selects Other, a required text box appears.

    • The system provides a required long text box to enter recommendations. The user must enter a minimum of 20 characters in the text box.

  11. In the Student Acknowledgement of Summary of Performance Participation section, the system displays the following information:

    • The system populates the student's full name from the Student Info/Demo Part B profile.

      • In the Student Signature area, the student's signature will appear through the digital signature functionality or the student can sign on a paper copy.

      • In the Date area, the signature date will auto-populate when the student signs digitally or manually enter a date when they choose to sign on a paper copy.

    • The system auto-populates the parent or guardian names that have Educational Rights selected in the Student Contacts Part B profile. The user can choose the parent or guardian who wishes to sign.

      • In the Parent/Guardian Signature area, a parent or guardian signature will appear through the digital signature functionality unless the user selects Parent/Guardian will sign a paper copy checkbox.

      • In the Date area, the signature date will auto-populate when the parent or guardian signs digitally.

  12. When the parent or guardian signs on paper, select the Parent/Guardian will sign a paper copy checkbox. When this checkbox is selected, the Document Delivery flyout section is required. The case manager will complete the Document Delivery section to indicate the consent response and date. This label and checkbox are suppressed on print.

  13. Click Save, Done Editing.

  14. Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

  • To add additional assessments, click the plus icon.

  • The user must enter a minimum of 20 characters in each text box.

  • To add additional recommendations, click the plus icon.

Document Delivery

This section is only required when the user selects Parent/Guardian will sign a paper copy checkbox. This section does not print.

  1. Within the Summary of Performance document, navigate to Document Delivery using the flyout menu.

  2. Select a Mode of Delivery.

  3. Enter a date manually or select a date using the calendar icon in the Date of Delivery field.

  4. Click Save, Done Editing.
