Transportation Eligibility Worksheet ASP - ESY

This page only displays when the Students Accessing Specialized Program (ASP) to Access FAPE option is selected as an ESY-related transportation eligibility category for the student in the Transportation section of the Extended School Year section of the IEP.

Transportation Eligibility Worksheet ASP - ESY

  1. Within the IEP document, navigate to Transportation Eligibility Worksheet ASP ESY using the flyout menu.

  2. In the Student Information section, the system auto-populates the following information:

    • From the Student Info/Demo Part B profile: 

      • Student

      • USI

      • DOB

    • From the LEA Entry/Exit Part B profile:

      • Grade

    • From the Special Education Part B profile:

      • Last Eligibility Meeting Date

      • Last IEP Annual Review Meeting Date

  3. The system provides two long stylized text boxes to describe and analyze student-level data for the considerations. The user must type a minimum of 20 characters in each text box.

  4. In the Eligibility Determination section, select a response for the Based on the considerations above, does the student require specialized transportation to access a free appropriate public education (FAPE) based on their placement in a specialized program? checkbox.

    • If the user selects Yes, the student is eligible for special education transportation services under the ASP category. A long stylized text box appears to provide a written justification of the IEP team determination, including information related to the student’s mode of transportation. The user must type a minimum of 20 characters in the text box. Upon finalization of the approved IEP, the data flow process selects the ESY Transportation Eligibility and the ESY Transportation Type ASP checkboxes in the student's Special Education Part B profile.

    • If the user selects No, the student is not eligible for special education transportation services under the ASP category. If appropriate, the IEP team may consider eligibility under structured transportation supports (STS) or medically fragile students (MFS) categories. Upon finalization of the approved IEP, the data flow process does not select the ESY Transportation Eligibility and ASP checkbox in the ESY Transportation Type field on the student's Special Education Part B profile.

  5. In the Mode of Transport section, select a response for Is public transit appropriate for this student? checkbox (This section appears when you select Yes for the Based on the considerations above, does the student require specialized transportation to access a free appropriate public education (FAPE) based on their placement in a specialized program? checkbox).

    • If the user selects Yes, the student is eligible for a metro fare card for public transportation through Metrorail or Metrobus.  

    • If the user selects No, the student is eligible for OSSE-DOT transport. 

  6. Enter a date in the Date of determination field. Upon finalization of the approved IEP, this date will flow to the ESY Transportation Eligibility Determination Date field on the student's Special Education Part B profile.

  7. Click Save, Done Editing.
