IEP: Educational Services & Placement Section

Purpose:
This update introduces a long text area for Other Services, similar to the existing functionality in Related Services – Outside General Education, allowing users to enter and style detailed information.

Key Updates

  • A long text area has been added for Other Services, enabling users to enter data and apply text styling using the available options.

  • The entered data is also displayed correctly in the IEP Snapshot section in both view and print modes.

Form Updates - NOTIFICATION OF CONFERENCE

This update aligns the Notification of Conference form with the latest state guidelines, ensuring clarity, compliance, and correct documentation for IEP meetings.

Key Updates

  • Removed: Entire line with initials for “Parent waives the 10 calendar day notification requirement.”

  • Updated Description:

    • Removed: “In order to discuss the educational needs of your child, you are invited to attend an Individualized Education Program (IEP) conference meeting to be held:”

    • Added after Dear Parent:
      “You are invited to attend an Individualized Education Program (IEP) conference meeting to discuss the educational needs of your child. The meeting will be held:”

  • Updated Notification Timeline:

    • Previous: “No later than 3 school days prior to the scheduled meeting”

    • New:
      “No later than three school days prior to the scheduled meeting, the school district must provide you copies of all written material that will be considered by the IEP team at the meeting so that you may participate in …”

  • Purpose Section:

Parent/Guardian Reeval Consent Forms - Consent for Eval: Needed Assessments section

This update aligns the Needed Assessments section of the Parent/Guardian Reeval Consent Forms – Consent for Evaluation with the latest state guidelines, ensuring accurate structure, labeling, and data capture.

Key Updates

  • Student Name: Shifted and aligned with the date field row as per the attached reference document.

  • Description Updated:

    • Old: “This form must be completed by the IEP Team.”

    • New: “This form must be completed by the Individualized Education Program (IEP) team.”

  • Domain Column: Removed Functional Performance and Cognitive Functioning.

  • New Field Added:

    • General Intelligence Data: Captures cognitive ability, including how the child takes in, understands, and expresses information.

    • Existing database fields may be used, or new fields may be created as required.

  • Functionality mirrors other options in the section.

  • Enabled Stylized properties:

    • Stylized Text

    • Allow Graphics in Stylized Text

  • Labels for Academic Performance and Communicative Status have been updated.

  • Stylized properties enabled for improved formatting and inclusion of graphics.

Parent/Guardian Reeval Consent Forms – Consent for Reevaluation

Purpose:
This update aligns the Parent/Guardian Reeval Consent Forms – Consent for Reevaluation section with the latest state guidelines (as per the attached document).

Key Updates

  • Updated the description:

    • Old:
      “The IEP team must complete page 2 of this form prior to obtaining parental consent for reevaluation. If the IEP team determines no additional evaluation is needed, then parental agreement and not parental consent is required.”

    • New:
      “The IEP team must complete the Parent Guardian Identification of Needed Assessments (ISBE Form 34-57B/C) prior to obtaining parental consent for a reevaluation. If the IEP team determines no additional evaluation is needed, then parental agreement and not parental consent is required.”

  • Updated label for PARENT/GUARDIAN CONSENT TO COLLECT ADDITIONAL EVALUATION DATA.

  • Changed checkbox description:

    • Old: “to collect the evaluation data as described on page 2 of this form.”

    • New: “to collect and/or review the evaluation data as described in the Parent Guardian Identification of Needed Assessments (ISBE Form 34-57B/C).”

New area added:

  • PARENT/GUARDIAN CONSENT TO WAIVE THE THREE-DAY DRAFT REQUIREMENT

    • Label description:
      “I understand that I am entitled to copies of all written material that will be considered by the team at a meeting to determine my child's eligibility for special education and related services and/or to review my child's IEP no later than three school days prior to the meeting. In situations where an eligibility determination and/or IEP meeting is scheduled less than three days in advance, my consent is required to waive this requirement. As the eligibility determination and/or IEP meeting will occur in less than three school days, I am providing written consent to waive the three-day requirement and provide the required documentation as soon as possible.”

  • Added two new checkboxes:

    1. Consent for eligibility documentation: Date/time documentation will be provided (date required & editable).

  • Verified that the date and Parent/Guardian Signature fields are available.

  • Updated the footer to match the state document.

    • New: “List the reasons and relevant factors for the above decision. (Provide a description of each evaluation procedure, assessment, record, or report the agency used as a basis for the proposed or refused action.)”

    • Previous: “If an evaluation was deemed appropriate or a reevaluation is necessary to determine”

    • New:
      “If an evaluation was deemed appropriate or a reevaluation is necessary to determine whether a child continues to be identified as a child with a disability, the process will begin upon the receipt of written informed consent from the parent/guardian. The Transfer of Rights Due to Age of Majority document has been updated to align with the latest state guidelines, ensuring compliance with current requirements.

Key Updates

  • Paragraph Replacement:

    • Replaced the paragraph beginning with “When a student with a disability reaches 18 years of age (the age of majority under state law)” with the new version as provided in the updated state document.

  • Notice Text Update:

    • Updated the line from
      “This serves as your one (1) year prior notice of the anticipated transfer of educational rights to the above-named student under IDEA.”
      to

  • New Statement Added:

The Agencies' Invitation to Transition Meeting form has been updated to align with current state guidelines, ensuring consistency and compliance.

Key Updates

  • Checkbox Section Update:

    • Added the label “CHECK ONE OR BOTH:” before the Parent/Student checkboxes.

    • Added the label “Parent/Guardian and/or Student who has reached the Age of Majority,” under the salutation area, where parents/students are addressed as “Dear”.

  • Agency List Update:

    • Under “THE SPECIFIC AGENCY/AGENCIES INCLUDE:”, rearranged the checkboxes as per the latest state document.

    • Added two new checkboxes:

      1. Department of Human Services (DHS)

      2. Military

  • Signature Section Update:

    • Replaced the existing field with School Personnel Signature and Title, specifying whether it is a lookup or non-lookup field.

    • Verified through unit testing that the staff signing process works as expected.

  • Footer Update:

    • Updated footer to ISBE 34-57L (8/25 - Content remains the same as the August 2015 version.)

Key Updates

  • Label and Paragraph Updates:

    • Updated paragraph descriptions to reflect the current state guideline terminology and improve readability.

  • Consent Area Update:

    • Modified the consent section text to match the updated wording as per the new state requirements.

  • Footer Update:

    • Updated footer to ISBE 34-57F (1/22).

The Goal section was updated to align with guidelines by revising descriptions, removing specific fields, updating the footer text, and ensuring data entered replicates in the IEP Snapshot while retaining the Report of Student Progress area.

  1. Updated description under "CONSIDERATION OF SPECIAL FACTORS" for supplementary aids and services.

  4. Updated description for behavior that impedes learning, including strategies and required forms.

  5. Added Yes/No checkboxes for accommodations during emergencies, with a long text field if marked Yes.

  6. Updated description under "LINGUISTIC ACCOMMODATIONS" for linguistic and cultural needs.

  7. Removed checkboxes with descriptions for services provided in a language other than English.

  8. Provided the full name for ASL (American Sign Language) and checked the order of the checkboxes.

  9. Updated description under "SUPPLEMENTARY AIDS, ACCOMMODATIONS, AND MODIFICATIONS" for additional aids and accommodations.

  10. Added header "EXTRA CURRICULAR AND OTHER NON-ACADEMIC ACTIVITIES" with a long text field for required accommodations.

IEP - Assessment accommodations section update

The Assessment Accommodations section in the IEP form has been reorganized to align with state requirements, with adjustments to section order, descriptions, headers, and accommodation terminology while maintaining existing functionality and data accuracy.

This section has been retired to accommodate the new changes.

New IEP section - WIDA Alternate ACCESS

A new section titled WIDA Alternate ACCESS has been added to the IEP document.

This section appeared only when in the Assessment Accommodations section:

  • “Yes” is marked under State Assessment of English Language Proficiency, and

  • Either of the following checkboxes is selected:

    • Participate in the alternate ACCESS with no accommodation(s), or

    • Participate in the alternate ACCESS with accommodation(s).
