This section ensures transparency, accountability, and a shared understanding among all stakeholders involved in the student’s special education plan.
This section includes:
A list of all participants who attended the meeting, along with their roles.
A summary of meeting discussions, including evaluations, student progress, accommodations, and any concerns raised by team members.
Any decisions made regarding eligibility, services, goals, and placement.
Parent/guardian input and any questions or concerns they have expressed.
Any action items or follow-ups required after the meeting.
By documenting these details, the IEP Meeting Notes and Participants section ensures that all team members, including educators, service providers, and parents, have a shared reference for the student’s support plan and future educational decisions.

This section shows the basic information required to identify the student.

Based on evaluation data, provide a statement of the student's present levels of academic achievement and functional performance, including how the student's disability/suspected disability affects the student's involvement and progress in the general education curriculum, or for children in early childhood, participation in appropriate activities.

The section captures the analysis of the tests and evaluations conducted.
For both the Cover Page and Notes section, the details can be captured either as written text in the space provided alongside the field. The text field also supports Spell Check on the inserted text.

CCC Team has reviewed existing data and has determined
This subsection documents decisions made by the Case Conference Committee (CCC) regarding whether additional assessments are needed or if existing data is sufficient to support educational planning.
This section provides four options to choose from:
Information is needed to reestablish eligibility for special education and related services. (reevaluation must occur by the next annual CCC meeting).
Information is needed to determine whether the student is eligible for special education under a different or additional eligibility category (50-day timeline).
Information is needed to inform the student’s case conference committee of the student’s service needs, such as the need for assistive technology or a related service (50-day timeline).
There is no need for reevaluation information.

To move a row upwards in the list, use the

You can delete an existing row by clicking theicon.
