Service Plan - Eligibility Determination
The Eligibility Determination section within the Service Plan is used to document whether a student qualifies for special education services under the Individuals with Disabilities Education Act (IDEA).
This section serves as a formal record of the eligibility determination process, ensuring compliance with IDEA regulations.
Student Information
This section shows the basic information required to identify the student.
The following table shows the fields that are displayed on the screen.

Eligibility Determination
This section prompts you to choose the result of the committee decision about the student's disability adversely affects the student's educational performance or not.
You must indicate your choice by selecting a Yes or No checkbox.
If you select
Yes - The Committee has determined the student's disability adversely affects the student's educational performance.
, then the workflow proceeds you to select the disabilities.The system allows you to select up to Five distinct disabilities.
Thereafter you should provide a detailed observational summary in the notes section to elaborate the chosen disabilities.
If you select
No – The Committee has determined the student's disability does not adversely affect the student's educational performance
, then the workflow skips the selecting the disabilities and prompts you to provide only the detailed observational summary in the notes section.
Reasons for eligibility or ineligibility determination
Based on evaluation data, provide a statement of the student's present levels of academic achievement and functional performance, including how the student's disability/suspected disability affects the student's involvement and progress in the general education curriculum or for children in early childhood, participation in appropriate activities.
Describe the reasons for eligibility or ineligibility determination, including all options considered and reasons these options were rejected.
The section captures the analysis of the tests and evaluations conducted.
For both the above sections, the details can be captured either as written text in the space provided alongside the field. The text field also supports Spell Check () on the inserted text.

Disability Information
In this section, you can record the primary and secondary disabilities identified in the student’s evaluation. This section allows you to capture up to five levels of disabilities.
