Transition Profile | PowerSchool Special Programs

A Transition Profile focuses on preparing students for life after high school.

It ensures that students with disabilities receive the support they need to transition successfully into post-secondary education, employment, and independent living.

Consideration Requirements

A student is eligible for a Transition only if:

The age is 14 or more.
The aggregate score is 8 or more.

However, in an exceptional case, either or both constraints might be relaxed.

Enabling Transition Profile within IEP Profile

The following table outlines the details of all the fields required to build a Student Transition Profile.

Integration IDT	Individual Development Team (IDT): An IDT is typically a team of professionals that may include teachers, special educators, counselors, psychologists, and sometimes the student’s family members. They work together to design, implement, and monitor a student's educational plan, especially for those who have unique educational needs. The team is a part of the process of developing an IEP (Individualized Education Program) or a 504 Plan for students with disabilities. SSID	State Student Identifier (SSID): The SSID is a unique identification number assigned to students within a state’s education system. It helps track a student’s educational data across different schools, districts, and years. It ensures that the student’s information is consistently tracked and helps manage the student’s progress, including those with an IEP or in Special Education programs. Enrollment Start Date	The date on which the student was enrolled in IEP.	In	Pulled from the student’s profile.
