SPEDSM-34807	504 Plan	When creating a new 504 Plan from an existing 504 Plan, data from the following fields will not be copied to the newly created form: Meeting Date Parent Contact Info Meeting participants Consent information Written Notice Office Use Only Added the Safeguards section as a new flyout in the form.
SPEDSM-34628	504 Plan	1. Corrected an issue where the user could not see the button for the reviewer to acknowledge the form. 2. Modified the plan as follows to correct the Review/Acknowledge feature: Newly created 504 Plans after this release will display the “Office Use Only” flyout. The ability to edit a final document is removed as well as the Office Use Area that appears at the bottom of the page. Forms in draft or review mode at the time when the release is deployed will still allow the user to edit the office use only and flyout. Forms that were finalized prior to the release, will have the ability to edit the form. Those forms will not have the flyout.
SPEDSM-34816	Administrative Data Sheet	Added text boxes next to three of the "If 18 Years or older" checkboxes. The text boxes are required when the respective checkbox is selected.
SPEDSM-20755	Attendance Sheet	Updated the form to include all four parent/contacts if the “Receives Letters” checkbox is selected.
SPEDSM-33179	Chapter 688 Student Referral (REF688)	Made the following updates to the Chapter 688 form: Retired the existing template and created a new template. Added non-lookup to the “LEA Contact Person” field. Changed the “School District (LEA)” field to flow from Responsible District. Changed the “LEA Address” field to flow from School District. “Name of High School” is now a lookup. Changed “School/Educational Placement” to "Servicing School" and flows the student's "Servicing School" from the profile. Address now flows in from Servicing School. Added DocuSign for the “Signature of Special Education Director/Designee”. Removed the Signature of Special Ed Director as the user will sign the paper copy or sign using DocuSign. Changed the “Phone” field to be an optional field. Shortened the signature line.
SPEDSM-34876	Extended Evaluation Form	Added text boxes next to three of the "If 18 Years or older" checkboxes. The text boxes are required when the respective checkbox is selected.
SPEDSM-32518	Forms (Notice of Proposed Action)	On the IEP > IEP Amendment and Evaluation Consent form, changed the order of the sections to display the Notice of Proposed Action first within the form.
SPEDSM-18579	Gifted IEP	Upon creating a Gifted IEP, the form will now flow in the latest Gifted Invitation meeting invitees and meeting date into the form. Added "Mobile Phone" under Phone. Changed “Parent Name” to "Parent/Guardian Name".
SPEDSM-32905	Gifted Invitation Letter	Added non-lookup to the meeting location.
SPEDSM-32379	IEP	Updated the form as follows: When the student’s age is 16 years or older as of the IEP meeting date and the Age of Majority standalone form does not exist for the student, the IEP will prompt to remind the user to create the Age of Majority standalone form. On the Administrative cover page, added text boxes next to three of the "If 18 Years or older" checkboxes. When creating a new IEP from an existing IEP, data from the following fields will not be copied to the newly created form: IEP Date ranges IEP Meeting Date Service Delivery start and end dates Additional info section or the Notice of Proposed Action section PL1 and PL2 sections Progress Reporting data. Additional Information: When the user selects the checkbox labeled "Document efforts to obtain....", a required text box is displayed. In the IEP Attendance Sheet, the team members from the latest (finalized or draft) invitation letter will flow into the form.
SPEDSM-32380	IEP Amendment	Added text boxes next to three of the "If 18 Years or older" checkboxes. The text boxes are required when the respective checkbox is selected.
SPEDSM-39342	IEP at a Glance	Added the second disability. Removed Least Restrictive Environment (LRE) as it is indicated in the Placement forms.
SPEDSM-40525	IEP Autism section	Removed text at the top of the form that references state regulations.
SPEDSM-36169	IEP Goals	Updated the IEP goal page. The goal page will now display only drop-down values where InUse = Yes.
SPEDSM-37631	IEP Placement Consent	Updated the Placement Consent forms within the IEP. Changed the database fields so that they are not shared by both sections.
SPEDSM-34812	IEP Placement PL2	Updated the PL2 form within the IEP. Users can now select the Educational Environment drop-down.
SPEDSM-37379	Keyword Tables	ProgramCodes and SPGoalFocus keyword tables are now admin-editable.
SPEDSM-28284	Medicaid Notice	Users can now create the form in bulk.
SPEDSM-39285	MedicaidProcedurecodes keyword table	The keyword table is now admin-editable.
SPEDSM-40534	SLD Determination	Updated the form removing any data flow to the student profiles.
SPEDSM-33336	SLD Determination	The parent/guardian names will now appear when the “Receives Letters” checkbox is selected. The form is now available for DocuSign.
SPEDSM-34801	Student Search by State ID	The user can now also search for a student using StateID (Search > Students).
