Assistive Technology and Adaptive Equipment Application

Fixed the following issues with the Assistive Technology and Adaptive Equipment Application:

• Updated the "# of Submissions" field under Student/School Information to include values 1-15.

• Added the text "TO BE COMPLETED IN CONSULTATION WITH" beside the Statement of Costs/Financial Request.

• Updated the "What is the student's primary learning environment" so that only one checkbox is mandatory. Previously, both checkboxes were required to be selected.

• Added a row for "Total Approved Amount" to the Application Review - District Staff section of the document.

SPEDSM-34187

Personalized Learning Plan

Updated the PLP-Adjusted Curriculum section of the Personalized Learning Plan. The course displayed is now read-only since it flows from the Justification Statement section. Also removed the Read button near the course window where a tooltip would appear.

SPEDSM-32004

Personalized Learning Plan

Updated the Personalized Learning Plan so that when a teacher is added to the Curriculum section, the teacher's name will appear on the Signatures page of the document.
