  Validation has been implemented to ensure that a valid 10-digit phone number is entered, along with the required country extension (+1).

  • Supports 4 current teachers, including both lookup and non-lookup options.

• Other Signatories

  • Includes School District Representative signature (with staff role).

  • The signatures of other individuals can be configured in the repeating row section.

     • Checkbox with label Grade (From-To) – converted into a Dropdown with Grade From and Grade To fields; appears on UI when checkbox is selected – Required

     • Early Entrance: Checkbox with Yes/No options
