Fixed an issue where the Census Data profile comments would print on a separate page.

Added “Prior Notice and Consent for Initial Provision Form” to the Developmental Delay packet.

Removed phone number from the following sections of the Eligibility packet where Meeting Participants are listed:

• Autism Spectrum Disorder
• Communication Disorder
• Deaf/Blindness
• Deaf or Hard of Hearing
• Intellectual Disability
• Orthopedic Impairment
• Other Health Impairment
• Specific Learning Disability
• Traumatic Brain Injury
• Vision Impairment

Removed the following section actions since they are related to a document that is no longer in use:

• Go to Referral Stage
• Meeting notes

Made minor changes to the text in the "Deaf or Hard of Hearing" section:
"1. The team has reviewed existing information...."

Fixed an issue where staff entered through the “Non-Lookup” option under the Assessment Grid could not save on “Save, Done Editing.”

Added the following sections to the Evaluation Planning document:

• Teacher Input Form for ASD
• Regional Fine/Gross Motor Referral File Review

Added staff email to the Signature section of the Evaluation Planning document.

Fixed the following formatting issues in the GIPREC, IIPLAN, SSTR, PARINV, and ParInvELMS documents:

• Some sections had dark borders, while other sections had light borders. The borders are now uniform across all sections.
• Aligned all Text and Checkboxes.
• If a label is clicked, the cursor will be placed on the corresponding field, making data entry quicker and easier.
• Made all the font sizes consistent.
• Corrected various printing issues. For example, sometimes borders would not print, pages would move off, etc.

• Updated the following changes to "Oregon IEP" and "School Age Forms":
  • Removed the "Notice of Transfer of Parents Rights" section from all new forms.
  • Added the following sections:
    • "Notice of Proposed Transfer of Sp Ed Rights"
    • "Notice of Transfer of Sp Ed Rights"

• The system will now provide a reminder to complete the "Notice of Proposed Transfer of Sp Ed Rights" if an IEP document is created and the student is 16 years old as of the IEP meeting date but will turn 17 within one year minus one day from the IEP meeting date. This reminder will appear only if the "Notice of Proposed Transfer of Sp Ed Rights" has not been included in the School-Age Forms.
• The system will provide a reminder to complete the "Notice of Transfer of Sp Ed Rights" if an IEP document is created and the student is 17 years old as of the IEP meeting date but will turn 18 within one year minus one day from the IEP meeting date. This reminder will appear only if the "Notice of Transfer of Sp Ed Rights" has not been included in the School-Age Forms.

Fixed an issue where there was no space between the State and Zip Code fields when using the “Lookup” feature to enter a school address.

Updated the following sections to display relevant data and to be editable where needed:

• International ELL/ESOL Data
• International Stdnt Demographic/Academic History

Updated the SupplementaryAidsMod keyword table:

• Moved some values to the bottom of the list so that the sequence improves readability.
• Changed the 'Acc' column value from "Yes" to "No" for the following keywords:

  • Yoga
• Added the following new keyword values:

• The Annual Review IEP Due Date field is now user-editable.
• The Amendment Date(s) no longer appear.

Updated all packets containing a Special Education Team Meeting Notice, so that meeting invitees have the lookup/non-lookup feature.
The updated packets include:

Updated the “School or Program Name” field to display the Attending School instead of the Home School.

Fixed an issue in the Special Education profile ESY Services section where the “ESY Services will be provided for this student:” checkbox is still selected as “No” (due to a former IEP) even when a new IEP is finalized. It now has the IEP ESY Decision section where the “To be determined” checkbox is selected, and the date is filled in.

Updated to import students' Legal Names from PowerSchool SIS into the Student Profile instead of Preferred Names. However, if PowerSchool SIS does not have a legal name for a student, it will import the Preferred Name instead.

Added a new Special Programs field within the Demographics/Enrollment section. The new field is a drop-down in the Enrollment Information section and uses the SpecialPrograms keyword table to provide the following options:

• LRC
• LRC-2
• Life Skills
• Social Communications
• RISE
• OASIS
• Post High

Added a new TBI Eligibility document to the system. This document contains the following sections:

• Traumatic Brain InjuryNote: Moved this section from the "Eligibility" packet to this new packet.
• Credible History InterviewNote: This is a new section.
• Credible History ReportNote: This is a brand-new section.
• Authorization/Protected Health Information
• Medical Statement or Health Assessment (Rev 3/14)
