SPEDSM-106917	Consent for Sharing and Billing Medicaid	Updated the Consent for Sharing and Billing Medicaid form to the latest version shared by the state as of August 2024. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-96627	Disability Categories Keyword Table	Added the following keywords to the Disability Categories keyword table: Emotional-Behavior Disability (KW Code - EBD) Once the Emotional Behavior Disability Eligibility Determination form is finalized, the Disability field in the Special Education Information section will be updated to 'Emotional-Behavior Disability'. Changes will be applied only to newly created documents. Speech Language Impairment (KW Code - SLI) Once the Speech Language Impairment Eligibility Determination form is finalized, the Disability field in the Special Education Information section will be updated as 'Speech Language Impairment'. Changes will be applied only to newly created documents. Existing student profiles with disabilities as emotional disturbances will remain unchanged. The student profile will be updated with new keywords as necessary.
SPEDSM-104275	FBA and BIP	Resolved the issue within the FBA & BIP form revision, where the staff was unable to sign the document as the previous signature date had not been cleared. The form has been corrected so that the Revision document can be sent to all the participants to sign digitally. The latest signature date will be captured on the form, and the PDF will be generated.
SPEDSM-110015	IEP - Meeting Participants Signatures	Added a new configuration, “Exclude Student Contacts, Adult Student from Digital Signature (if attended by other means),” to the Globals profile > Main Flyout > Configurable Option > IEP section. Globals profile can be accessed from the Adminstration menu > Configuration > Profile Types> select Globals. The configuration determines whether the parents/guardians/ adult students who attended the IEP meeting by other means, such as phone, video conference, etc., will be excluded from digital signature submission. By default, the configuration is set to "Yes." The system will not include the parent/guardian/adult student attending the IEP meeting by other means (not in person) for digital signature. The configuration should be set to No for schools that require the parent/guardian/adult student to digitally sign the document even when the meeting is attended by other means (online, phone, and so on). Once the global configuration changes, refresh the existing IEP forms using the document options - More > Update from Student Profile. This ensures expected behavior per the global configuration. The configuration only applies to parents, guardians, or adult students. All staff or Minor Students added as IEP Meeting Participants & attending the meeting can digitally sign the document regardless of the attendance option.
SPEDSM-106919	Individualized Education Plan	Updated the Individualized Education Plan(IEP) to the latest version shared by the state as of August 2024. In the IEP > Assessment Addendum section, the PEEP/KEEP assessments are optional, allowing users to save the section without adding a PEEP/KEEP assessment. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-106914	Individualized Education Plan - PWN section	Updated the PWN section of the IEP form to the latest version shared by the state as of August 2024. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-104351	Invitation to a Meeting	Resolved an issue that listed only the Parent1 & Parent2 contacts without checking the Receives Letters indicator. The form is corrected to list the parent/guardian/surrogate student contacts with the Receives Letters flag selected.
SPEDSM-85006	Medical Profile and Consent for Medicaid Form	Made the following changes to the Medical profile: Removed Medicaid Eligibility and Medicaid ID Number from the profile. When the Consent for Medicaid is finalized, the Medicaid Consent Date will be updated along with the parent response as Yes/No in the Consent Response field.
SPEDSM-106918	Prior Medical History	Updated the Prior Medical History section to the latest version shared by the state as of August 2024. Applied changes to the following Eligibility Determination forms: Autism Multiple Disabilities Other Health Impairment Orthopedic Impairment Traumatic Brain Injury The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-86001	Prior Written Notice of Revocation of Consent for Special Education and Related Services	Updated the Prior Written Notice of Revocation of Consent for Special Education and Related Services form to the latest DOE version. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-106912	PWN and Consent for Eval/Re-Eval	Updated the PWN and Consent for Eval/Re-Eval form to the latest version shared by the state as of August 2024. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-106916	PWN of Change of Placement	Updated the PWN of Change of Placement form to the latest version shared by the state as of August 2024. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-106915	PWN Review of Existing Data	Updated the PWN Review of Existing Data form to the latest version shared by the state as of August 2024. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-108677	Student Records Transfer Request	Updated the form, allowing users to select the “To” and “From” schools to transfer student records. The form uses the Location data for lookup.
SPEDSM-106913	Developmental Delay Eligibility Determination Form	Updated the Developmental Delay Eligibility Determination form to the latest version shared by the state as of August 2024. A new checkbox is added to indicate No Delay under Standard Deviation. The changes will only apply to newly created documents, while all existing documents will remain in the previous format.
SPEDSM-110646	Keyword Table - GenderTable	The keyword table, GenderTable is updated to support the language translations. Added a new column Description. The standard translation procedures can be used to translate the Gender in any document template being translated.
SPEDSM-94469	Consent for Sharing and Billing Medicaid	An issue with the Consent for Sharing and Billing Medicaid form is to capture the Parent Consent response when the form is signed digitally. When the document is signed using PowerSchool Digital Signature, the first parent’s consent response & the consent date/signed-on date will be updated on the Medical profile. If the document is signed using DocuSign, the last parent/guardian’s response and signature date will be updated on the profile. Due to limitations on DocuSign, the first signee’s response can not be captured. The staff/case manager is expected to update the consent response and date on the form using the Office Use Only section or update the profile details. If the staff/case manager edits the final document for Parent Consent and Consent Received Date in Office Use Section, on save, the details will be updated to the profile.
