Consent For Release Of Records/Student Records Transfer Request

Revised the Consent Student Records form to match the latest updates to the State form.

Renamed Consent For Release Of Records form to Student Records Transfer Request. The updated form will only be available for newly created documents, while all existing forms will remain in the previous format.

With the newly revised form, the system allows the user to create a request for release of records of given student, for whom the document is being added.

Developmental Delay Eligibility Determination

Made the following changes to the Developmental Delay Eligibility Determination form:

• Option fields for Percentile and SD are now available. Users can leave these fields unselected and still capture assessment details and save/finalize the form in case of no delay.

• Similar to other eligibility documents, a checkbox is added to mark an assessment area as N/A or Not Applicable.

Eligibility Determination Forms

• Autism Evaluation Summary Report and Other Health Impairment Eligibility Determination forms—The checkbox for the Student’s Medical History is required to save and finalize.

• On the Hard of Hearing/Deafness Eligibility Determination form, Areas of Evaluation are required for Language and Growth, Speech and Language, & Academic Achievement. Users should provide at least one assessment detail under each area.

Eligibility Documents and IEP

Made the following changes on all Eligibility documents for participant’s Digital Signature:

• On all Eligibility Documents, when Participated via telephone, video conference, or other means is selected, the parent/guardian or student who is an adult does not need to sign the document digitally. Thus, the Parent/Guarding/Student will not be listed to submit for digital signature.

• Marking Participated via telephone, video conference, or other means, counts as a signature and attendance, so a digital signature will not be created if the checkbox is checked. 

Made the following changes to IEP to capture the digital signature of the participants:

• On IEP Meeting Participants section, for all parents/guardians (in case of minor students) or adult students, the attendance option selected is Attended Other Means, the user must select the checkbox for Participated via video conference, in order to save and finalize the document. The system will show a validation message for the same.

• Similarly, for all parents/guardians (in case of minor students) or adult students, the attendance option selected is Not Attended, the user must select the checkbox Did not attend, to save and finalize the document. The system will show a validation message for the same.

• If parents/guardians/student attended the meeting by another meeting and not in person, they will not be required to sign digitally and will not be listed for digital signature.

• Parent/guardian/student with attendance option as In Person will sign the document digitally.

• Participants, other than parents/guardians, will sign the document digitally, irrespective of the attendance option chosen.

Invitation to a Meeting

Updated the form to address the notice to the parent/guardian/surrogate who receives letter. The system will verify the "Receives Letter" attribute added to the student contacts on the Demographics profile and will address the notice to the respective student contacts with "Receives Letter" checked.

Keyword Table Updates - Service Time Ranges, Educational Settings and Educational Environments

Updated the descriptions of the various keywords to have Code as a suffix to the existing keyword description. The keyword tables updated are:

• ServiceTimeRanges

• EdSettings

• EdEnvironmentTable
