Section 504 Forms

Surrogate Parent Inclusion in Section 504 Documents

• Updated logic to ensure the Surrogate Parent is included as a meeting participant whenever “Receives Letters” is marked as Yes.

• This update applies to the following Section 504 documents:

  • Section 504 Meeting Notice and Invitation

  • Written Notice/Consent

  • Section 504 Eligibility

  • Section 504 Plan

  • Section 504 Manifestation Determination Review

  • Authorization for Release/Exchange of Information

• In general, all Section 504 category documents will now include the Surrogate Parent along with Parent/Guardians, as long as they are eligible to receive student notifications.

• Digital Signature fields have been added for the Surrogate Parent, where applicable. The user can choose whether the Surrogate Parent will sign the document.

• These changes apply only to newly created documents after this release.

State Form Compliance Updates: 2025–26

The forms listed below have been updated to align with the 2025 Utah State Board of Education Model Forms updates. The forms in the system now match the latest state-issued formats to ensure full compliance.

PS SIS Integration - Student Demographics

The Student Data Import layout for PowerSchool SIS has been updated to include Student Email, which will now be imported into the Student Demographics Profile in PSSP.

IEP - Meeting Minutes

The user can now add additional participants to the Meeting Minutes section of the IEP, and they will be copied automatically from the Team Members section. This includes Parent/Guardian participants with "Received Letters = True" and any other members you add. When the user updates and saves the Team Members list, the Meeting Minutes section will reflect these changes after the user uses the "Add new meeting minutes" action. The option to manually refresh by copying from the IEP Team Meeting Participants is still available if needed.

This update ensures all meeting participants are accurately included in your Meeting Minutes, saving user’s time and reducing manual entry.
