Service Plan

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Private School Service Plan under Service Plan, and click Go.

  4. The system will check if the student is Eligible for Special Education. If the student has a Special Education Exit Date on the Special Education Part B profile, the system will not create the document. If the student is not eligible for special education which is identified on the Special Education Part B profile Initial Eligibility Information>Eligibility Outcome or Eligibility Determination>Eligibility Outcome, the system will not create the document.

  5. Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.

  6. The system displays all available sections for the Service Plan.

  7. In the Student Information section -

    • The system auto-populates the following information:

      • From the Student Info/Demo Part B profile: 

        • Student

        • USI

        • DOB

      • From the LEA Entry/Exit Part B profile:

        • Grade

      • From the Special Education Part B profile:

        • Last IEP Annual Review Meeting Date

        • Last Eligibility Meeting Date

      • From the Special Education Part B profile:

        • Initial Evaluation Date

        • Re-evaluation Due Date

    • Enter a date manually or select a date using the calendar icon in the Service Plan Meeting Date field. Upon finalization of the approved document, the date will flow to the Service Plan Completion Date field on the Special Ed Part B profile.

    • The system auto-populates the Annual Review Meeting Due Date.

  8. Complete the information on the screen within the following sections:

  9. Click Save, Done Editing.

  10. Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

Eligibility Category

  1. The Primary Disability value will flow from the Special Ed Part B profile.

  2. The Secondary Disability value will flow from the Special Ed Part B profile.

Location of Services

Select a location from the drop-down list. When the user selects the value Virtual, the system displays a button to attach a file and a message: Please attach a copy of a signed Consent for Telehealth. 


The system provides a series of checkboxes to confirm the purpose of the service plan meeting. The user can select only one checkbox.

Meeting Participants

  1. The system auto-populates the student's name and any student contact that has Educational Rights selected on the student's Student Contacts Part B profile. To add each parent or guardian as a meeting participant, the user must select the checkbox next to each parent or guardian's name. If the student has the right to be added as a participant, the user must select the Student checkbox. When the student checkbox is not cleared, the student name does not print or display in the non-edit mode.

  2. Select an LEA Representative using the lookup or non-lookup link.

  • To add more participants, click the enter icon.

  • Once the student contact is updated in the Student Contacts Part B profile, the unfinalized documents display the latest parent or guardian name with education rights. The system auto-selects the latest Parent/Guardian name when the document is accessed using the More, Update Document from Student Profile option.
