Section 504 - Evaluation and Determination

This page outlines the process that involves a comprehensive review of the student's educational records and any other relevant medical or psychological information. The evaluation aims to determine whether the student has a disability that substantially limits one or more major life activities and whether the student requires accommodations to have equal access to education.

Section 504 Evaluation and Determination of Eligibility

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Section 504 - Evaluation and Determination under Section 504, and click Go.

  4. Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.

  5. In the Student Information section, the system auto-populates the following information:

    • From the Student Info/Demo Part B profile: 

      • Student

      • USI

      • DOB

    • From the LEA Entry/Exit Part B profile:

      • Grade

    • From the Special Education Part B profile:

      • Last IEP Annual Review Meeting Date

    • From the Section 504 profile:

      • Section 504 Coordinator

  6. The system auto-populates the Referral Type from the Section 504 profile.

  7. Enter a date manually or select a date using the calendar icon in the Date of Section 504 Meeting field.

  8. Complete the information on the screen within the following sections:

  9. Click Save, Done Editing.

  10. Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

Meeting Participants

  1. Under Name, select a participant using the lookup function or type a free-form name, and click non-lookup.

  2. Under Title, the system populates the participant's title when the lookup is used. If the non-lookup is used, the user must enter the title.

The user can add multiple participants.

Data Reviewed

The system provides a series of checkboxes to determine the student's eligibility. The user must select the applicable checkboxes listed in the Check All That Apply area.

  • If the user selects Other, a required textbox appears.

Determination of a Physical or Mental Impairment

The system provides checkboxes related to a student's physical or mental impairment substantially limiting a major life activity. The user must select the applicable checkboxes listed in the Check All That Apply area.

  • When the user selects the A physical impairment checkbox, the user must select the appropriate checkbox under Physical Impairment. The system provides a long stylized textbox for the user to Specify the physical impairment.

  • When the user selects the A mental or psychological disorder checkbox, the system provides a long stylized textbox for the user to specify the mental or psychological impairment.

  • The user must complete the following fields:

    • Data Source: The system provides a text field to enter the data source.

    • Description: The system provides a text field to enter a description of the supporting documentation.

    • Date Completed: Enter a date manually or select a date using the calendar icon.

    • Document Uploaded: The system allows to upload of supporting documentation from the local folders.

    • Date Uploaded: The system auto-populates the date when the user uploads the supporting documentation.

Major Life Activity/Activities Affected by the Impairment

The system provides a series of checkboxes when a student has been found to have either a physical or mental impairment. The user must select the applicable checkboxes listed in the Check All That Apply area.

  • If the user selects Other, a required textbox appears.

  • Select the No major life activity is impacted by the impairment checkbox when a student's physical or mental impairment does not affect their life activities.

Substantial Limitation

Select a response to the question When compared to students without disabilities ..... perform the major life activity?

  • When the user selects Yes, the system provides a long stylized textbox labeled Documentation.

Eligibility Determination

The system provides a series of checkboxes to determine a student's eligibility. The user can select only one checkbox.

  • When the user selects the Not Section 504 Eligible checkbox, a long stylized textbox appears labeled Recommendations. Upon finalization of the document, when selected, the system will flow to the Section 504 profile, Student is eligible under Section 504, No checkbox.

  • When the user selects No in the Substantial Limitation section, Section 504 Eligible with Section 504 Plan checkbox is not available to be selected. Upon finalization of the document, when selected, the system will flow to the Section 504 profile, Student is eligible under Section 504, Yes checkbox.

  • When the user selects No in the Substantial Limitation section, Section 504 Eligible with Section 504 Plan and 504 Eligible for Transportation checkbox. Upon finalization of the document, when selected, the system will flow to the Section 504 profile, Student is eligible under Section 504 Yes checkbox and Eligible for Transportation Yes checkbox.

  • When the user selects No in the Substantial Limitation section, Section 504 Eligible with no Section 504 Plan checkbox. Upon finalization of the document, when selected, the system will flow to the Section 504 profile, Student is eligible under Section 504, No checkbox.

  • The system provides a long stylized textbox labeled Additional Notes and Comments to type details of any student who is found not eligible under Section 504.
