Section 504 - Referral

This page provides information on the evaluation of a student to determine whether they have a disability that substantially limits a significant life activity, such as learning. Referrals can be made by parents, teachers, or other school staff, and they typically involve a formal process of gathering information about the student's academic and behavioral performance.

Section 504 - Referral

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Section 504 - Referral under Section 504, and click Go.

  4. Enter a Label/Comment and click New. Click Additional Settings to specify the year and sections to include.

  5. In the Student Information section -  

    • The system auto-populates the following information:

      • From the Student Info/Demo Part B profile: 

        • Student

        • Student Address

        • USI

        • DOB

        • English Learner: The system displays Yes when the English Learner Proficiency checkbox is selected; else, the system displays No.

      • From the LEA Entry/Exit Part B profile:

        • School

        • Grade

      • From the Special Education Part B profile:

        • Last IEP Annual Review Meeting Date

      • From the Section 504 profile:

        • Section 504 Coordinator

    • In the Home Room Teacher field, select a name from the staff table using the lookup function or type a free-form name, and click non-lookup.

  6. Enter a date manually or select a date using the calendar icon. The date cannot be later than today's date and must be earlier than the date indicated in the Referral Date field.

  7. Complete the information on the screen within the following sections:

  8. Click Save, Done Editing.

  9. Finalize the document once completed. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

Referral Information

  1. In the Referred by field, select a name from the staff table using the lookup function or type a free-form name, and click non-lookup.

  2. In the Role field, the system populates the staff's role when the lookup is used. If the non-lookup is used, then the user will be required to enter the role.

  3. Enter a date manually or select a date using the calendar icon in the Referral Date field. When the document is finalized the date flows to the Section 504 profile.

  4. Select a Type of Referral. If the user selects Other, a required textbox appears.

  5. In the Staff Completing the Referral field, select a name from the staff table using the lookup function or type a free-form name, and click non-lookup.

To add more referral information, click the plus icon.

Previous

  1. Enter a date manually or select a date using the calendar icon in the Date of Previous Referral field.

  2. Select the Type of Referral Received.

School Data

  1. Select a response for the Is referred student's attendance a concern? checkbox.

    • If the user selects Yes, then the attendance fields become required.

  2. The system provides the following textboxes to enter student's attendance for the current school year:

    • Present

    • Absent

    • Tardy

Student Strengths

The system provides a series of checkboxes to indicate the student's strengths. The user must select at least one checkbox. If the user selects Other, a required textbox appears.

Reason for Referral

  1. The system provides a long stylized textbox to describe why the student is being referred for a Section 504 evaluation.

  2. Type a Data Source in the text field. 

  3. Enter a date manually or select a date using the calendar icon in the Date field.

  4. Type Student Performance or Behavior in the text field.

  5. Type Expected Performance or Behavior in the text field.

To add multiple data sources, click the plus icon.

Academic Records and Assessment Data

The system provides the following long textboxes:

  • Reading

  • Math

  • Written Language

  • Other

  • Additional Information

The user must complete one textbox and type at least 20 characters in the textbox.
