Child Outcomes Summary Form

  • The system prevents document creation if the student's Initial IEP meeting date and case manager name are missing from the Special Education Part B profile.

  • The user must ensure that the Program Exit Date is greater than the Program Entry Date.

Child Outcomes Summary (COS) form

  • A data collection tool used by early intervention and Early Childhood Special Education (ECSE) teams to summarize a child’s progress in three key outcome areas:

    • Positive social-emotional skills (including relationships)

    • Acquisition and use of knowledge and skills (including early language/communication)

    • Use of appropriate behaviors to meet needs

  • Uses a 7-point rating scale to indicate the child’s level of functioning compared to age-expected development.

  • Completed through a team-based decision-making process, incorporating multiple sources of data, such as observations, assessments, and input from families.

The Child Outcomes Summary (COS) form can be for the following two types:

  • Entry COS – Completed when a child enters an early intervention or ECSE program to establish a baseline of their developmental skills.

  • Exit COS – Completed when the child exits the program (or transitions to another service) to measure progress over time.

The only difference between the Entry COS Form and Exit COS Form is that the Exit COS shows a 7-point rating scale to indicate the child’s level of functioning compared to age-expected development.

Child Outcomes Summary (COS) report

  • A summary document or report generated from the data collected on the COS Form.

  • Provides an overview of a child's progress over time, often used for reporting at the program, district, or state level.

  • May include aggregated data from multiple children to analyze trends and program effectiveness.

  • Typically used by administrators, policymakers, and program evaluators for compliance and improvement efforts.
