Child Outcomes Summary Form (Exit)
The Child Outcomes Summary (COS) Exit form is used to evaluate a child's progress at the time of exit from an early intervention or Early Childhood Special Education (ECSE) program. This form measures developmental changes in the same three outcome areas assessed at entry:
Positive social-emotional skills (including relationships)
Acquisition and use of knowledge and skills (including communication)
Use of appropriate behaviors to meet needs
The COS Exit form allows teams to compare a child's current abilities with their initial entry ratings, providing insight into developmental growth and the effectiveness of program services. Like the entry form, ratings are determined through a collaborative approach utilizing multiple sources of information.
