Initial Evaluation Report
The Initial Evaluation Report summarizes the assessment results to determine a student’s eligibility for special education services. This report is crucial for identifying the student’s needs and developing an Individualized Education Program (IEP) if eligible.
To navigate to the report, click Reporting, Standard Reports, IDOE IEP Reports, and then select Initial Evaluation Report.
Selection criteria
Include students who have a disability according to IDEA.
Include students with an active and finalized IEP.
