Transition IEP Diagnostic Report
The Transition IEP Diagnostic Report provides a detailed overview of how well a student’s IEP (Individualized Education Program) supports their move from school to life after graduation. This includes plans for jobs, further education, and independent living.
The report helps schools:
Check if transition goals and services are included and appropriate
Make sure the required timelines are followed
Identify areas that need improvement to better support the student’s future
It’s a tool for educators and administrators to ensure transition planning in the IEP is complete, timely, and meaningful.
Submission requirements
To be included in the Transition IEP Diagnostic Report, the student must have an active IEP and be age 14 or older (or younger if transition planning has started). The IEP should include required transition components such as post-secondary goals, transition services, courses of study, and related annual goals. The report checks for missing or incomplete information in these areas, including whether the student and any relevant outside agencies were invited to the IEP meeting. The IEP must be in progress or finalized within the reporting period to be reviewed in the diagnostic report.
Generate report
From the Reporting menu, choose Standard report.
Click APR/SPPs Diagnostic Reports, then click Transition IEP Diagnostic Details.
Enter a Start Date and End Date for the reporting duration.
Click Update Report with Values.
