Section 504 - Authorization for Release/Exchange of Information

The Authorization for Release/Exchange of Information form under the Section 504 Documents list now supports PSSP Digital Signature, allowing parents, guardians, and staff to sign electronically.

Prerequisites:

• An active license for the PSSP Digital Signature package.

• Digital Signature configuration set to Enabled.

Scope of Support:

• Only forms created after the release will support digital signatures.

• Existing forms will not support digital signatures, even if the configuration is enabled.

Form Behavior When Digital Signature is Enabled:

• Lists Parents/Guardians marked as "Receives Letters" = Yes in the Parent/Guardian Contact Information profile.

• Lists students aged 12 years or older as eligible signers.

• Requires at least one Parent/Guardian or eligible Student to sign.

• Requires a Witness (staff) added via lookup or non-lookup options to co-sign.

• The form must be in Review or Final status before submission for digital signature.

 Section 504 - Written Notice/Consent

The Written Notice/Consent form under the Section 504 Documents list now supports PSSP Digital Signature, allowing parents, guardians, and staff to sign electronically.

Prerequisites:

• An active license for the PSSP Digital Signature package.

• Digital Signature configuration set to Enabled.

Scope of Support:

• Only forms created after the release will support digital signatures.

• Existing forms will not support digital signatures, even if the configuration is enabled.

Form Behavior When Digital Signature is Enabled:

• Lists Parents/Guardians marked as "Receives Letters" = Yes in the Demographics profile.

• Lists students aged 18 years or older as eligible signers.

• Requires at least one Parent/Guardian or eligible Student to sign.

• The form must be in Review or Final status before submission for digital signature.

• It populates the Date of Section 504 Consent to Evaluate on the 504 profile with the signature date of the parent, guardian, or student who signed the form first.

Section 504 - Manifestation Determination Review

The Manifestation Determination Review form under the Section 504 Documents list now supports PSSP Digital Signature, allowing parents, guardians, and staff to sign electronically.

Prerequisites:

• An active license for the PSSP Digital Signature package.

• Digital Signature configuration set to Enabled.

Scope of Support:

• Only forms created after the release will support digital signatures.

• Existing forms will not support digital signatures, even if the configuration is enabled.

Form Behavior When Digital Signature is Enabled:

• Lists Parents/Guardians marked as "Receives Letters" = Yes in the Parent/Guardian Contact Information profile.

• Lists students aged 18 years or older as eligible signers.

• Requires at least one Parent/Guardian or adult Student to sign.

• Allows 504 Coordinators/Designers added via lookup or non-lookup to sign electronically.

• The form must be in Review or Final status before submission for digital signature.

The 504 Plan under the Section 504 Documents list now supports PSSP Digital Signature, allowing parents, guardians, and staff to sign electronically.

Prerequisites:

• An active license for the PSSP Digital Signature package.

• Digital Signature configuration set to Enabled.

• Only forms created after the release will support digital signatures.

• Existing forms will not support digital signatures, even if the configuration is enabled.

Form Behavior When Digital Signature is Enabled:

• Lists Parents/Guardians marked as "Receives Letters" = Yes in the Parent/Guardian Contact Information profile.

• Lists students aged 18 years or older as eligible signers.

• Requires at least one Parent/Guardian or adult Student to sign the Consent to Implement Section 504 Plan.

• Allows Meeting Participants added via lookup or non-lookup to sign electronically.

• The form must be in Review or Final status before submission for digital signature.
