Completing the Referral Conference Decision Document

Select the Referral Conference Decision document link auto-generated as a superseding document to the Referral document.

The Student Information section of the document will auto-populate the following data and is read-only.

Select “Will Sign Paper Copy” if the parents will sign a paper copy of this document.
Will Sign Paper Copy
Select Will Sign Paper Copy if parental response and consent will be collected on a paper copy and not via electronic signature.
If the parental response will be collected electronically, refer to “How to sign a document through digital signature”

Upon creating of the document, the system will import in the Referral Date from the linked Referral document. The field is required. The user can edit the date value.

Use the date picker or manually enter the Referral Conference Decision Date.

Select the appropriate evaluation decision. Only one value can be selected.

Use the Up and Down icons to change the order. Use the X icon to remove a row entry.

Public Agency Representative value can be populated by using the lookup or non-lookup option.
For each participant, their Signature and Date of signing are recorded in this section.

Use the lookup or Non-lookup feature to select Public Agency Representative and enter the Date of signature (if Will Sign Paper Copy is selected, date is not editable).

Click Save, Done Editing.
If there are required fields not yet completed, the system will display the following error message:
