Revocation of Parental Consent

Revocation of Parental Consent for Placement refers to a parent or legal guardian’s written decision to withdraw consent for their child to continue receiving special education services.


  1. Open the student’s special education record and locate the Revocation of Parental Consent section.

    1. Check the pre-filled details; Student Name, ID, Date of Birth, Age, Grade, and School pull in from the Student Information profile.

  2. Select Will Sign Paper Copy, if the parent/guardian will sign on a paper copy. Leave the checkbox unchecked if it will be signed via the digital signature method.

  3. Enter revocation details: I, (select Parent or guardian name from drop down). The drop down will populate each contact that has Receives Letters checkbox checked.

  4. Save the document.

  5. Print the document on paper for the parent/guardian to sign OR from the ‘More…’ menu, select ‘Send document for digital signature’. This step has to be followed only when ‘Will Sign Paper Copy’ checkbox was not selected in step 2.

  6. The user can attach the signed paper copy that was scanned by clicking on Navigate To, File Attachments.

  7. Finalize the document.
