  • Included legal references.

  • Removed the following fields: 

    • Date of Initial Consent Received

    • Consent for Reevaluation Date 

  • Added a validation rule for the Most Recent Reevaluation Date to equal or greater than the meeting date. 

  • Added the ability for Digitial Signature.
