Digital Signature Reports for Document Status Tracking

To provide more visibility into the status of electronically signed forms, we’ve updated the existing DocuSign report and introduced a new companion report for tracking incomplete forms. A new report category, “eSignatures“ is added under Standard Reports.

1. Report: eSignature Status Report

The existing DocuSign Status Report has been enhanced and renamed to eSignature Status Report under the eSignatures category.

Key Changes:
  • The report now displays only students who have at least one e-signature form recorded (any column with data).
Students without any e-signature activity will no longer appear.

  • The report includes students regardless of DocHistory.

  • Only completed e-signature forms will be displayed.

2. Report: eSignature Incomplete Forms

A new report titled eSignature Incomplete Forms has been added under the eSignatures category.

Key Features:
  • Displays students and forms where the e-signature status is Sent, Delivered, Autoresponded, or Processing.

  • Includes students regardless of DocHistory.

  • Only forms that have been sent for signature (via DocuSign or PS Digital Signature) and are pending completion will appear.

  • Students with no pending e-signature forms will not be displayed.

These updates provide a clearer view of which forms are completed and which are still in progress, supporting better tracking and follow-up for both DocuSign and PS Digital Signature processes.

Referral Form Consolidation and Automated Status Tracking

To align with the Utah DOE SY 2025–26 model form updates, the referral workflow and related PSSP forms have been streamlined.

These enhancements ensure consistency, reduce form duplication, and support more accurate tracking for future reporting needs.

  1. Form Updates

    1. Retained Form

      1. Referral for Special Education Services (RefSpEdSrv)
This is now the only active referral form, consistent with the updated state model.

    2. Retired Forms

      1. Referral (REF)

      2. Referral for Evaluation for Preschool (RefPreSchl)

These retired templates may still appear in list reports (e.g., eSignature Status Report, eSignature Incomplete Forms) for historical records. They will remain available for reference, but will not be used going forward.

  1. New Profile Fields for Referral Tracking

  2. To support improved referral monitoring and future reporting, new data elements have been added to the Special Education Information Profile.

    1. New Field Added: Referral Status

      1. Located directly below the Referral Date.

      2. Dropdown field, non-mandatory.

      3. Values include: Approved, Rejected.

    2. Referral Date → Automatically saved to the student’s profile.

    3. Referral Status
Based on selections in the Referral for Special Education Services (RefSpEdSrv) document:

      1. If “Special education evaluation recommended” is selected → Status = Approved

      2. If “No special education evaluation recommended” is selected → Status = Rejected

Both the Referral Date and Referral Status will now be automatically updated via document data flow when the Referral for Special Education Services (RefSpEdSrv) form is finalized.

Bug Fix Summary

Group Evaluation Summary Report

A system fix has been implemented to resolve an issue that occurred when users attempted to send Group Evaluation Summary Report for digital signature.

Users were receiving an error message stating ‘No signers available’ when sending certain documents for digital signature, even when signers were correctly assigned.

The issue has now been resolved. Users can successfully send documents for digital signature without receiving the incorrect error message, provided that valid signers are configured for the document.
