This document is the acknowledgement by the Public Agency Authorized Representative after the Parent or the student has submitted by Revocation request.
To access this document - Open the Revocation Document for the student and click the fly-out menu, and click ‘Confirmation’.
Fill in the following details and save the document.

Use date-picker to mention the date on which the parents had sent the Revocation Request.

Use date-picker to mention the current date, or the date when the confirmation is being sent.

Use look-up to select the name of the Public Agency Authorized Representative.

Save document. Submit for digital signature from the ‘More…’ menu or proceed to send the document physically.
