Disability History

The Disability History profile stores the student’s disability value(s), both disability values in the past as well as present. The profile data should not be manually edited from the profile itself but rather it should be managed from the EPC or EDR documents.

Disability profile includes:

  • Primary Disability: Displays the student’s disability value.

  • Start Date: Stores the date the student was found eligible for the disability.

  • End Date: Stores the date the student no longer qualifies for the disability.

Data Flow

The following documents flow data to this profile:

Document

Description

Evaluation Programming Conference (EPC)

When the user selects “Evaluation data substantiates the existence of a disability consistent with state and federal regulations implementing IDEA” and selects a disability value. Upon finalization of the EPC document, the system will create a new Disability History profile record using the Date Provided to Parent value as the disability start date.

Evaluation Data Review (EDR)

When the user selects “Child meets IDEA eligibility” in section B., upon finalization of the EDR, the system will flow and create a new Disability History profile record if the existing disability in the profile is different than what is selected in the EDR document. If it is different, the system will end the existing disability in the profile to the day before the determination date on the EDR. A new disability profile record will be created with a start date using the determination date.
