Manifestation Determination Review (MDR)

Completing the Manifestation Determination Review (MDR) Form

  • The document pre-populates the student information: 

    • Student Name 

    • ID 

    • Date of Birth 

    • Age (calculated using the “Date of Decision” date value or today's date if Date of Decision is blank)

    • Current Grade/Placement

    • School

  • Use Date-picker to add the ‘Date’ value.

  • Use Lookup to add the ‘Public Agency’ name.

  • Select whether the student is Eligible for IDEA services?

    • If YES, state qualifying disability: Specific Learning Disabilities.

  • Add the List of relevant IEP Team members (by name and title).

If there are required fields not yet completed, the system will display the following error message:

Section was saved but is incomplete until all required fields (pink highlighted) are filled.

Add as many multiple rows of details as required.
To add every additional Service, click the image-20250218-141308.png icon and a new row will appear.

The user can also rearrange the sequence of the Service(s) list in any order.

  • To move a disability upwards in the list, use the up-w24h24.svg icon.

  • Likewise, to move a disability downwards in the list, use the down-w24h24.svg icon.

To delete a row, use the delete-w24h24.svg icon.

  • From the given options, select the Sources of relevant information under review.

  • Provide a text response to:

    • Description of the act of misconduct (include all relevant details).

    • Description of proposed disciplinary action.

    • Factors considered in making a manifestation determination (Use additional paper, as necessary, to respond to these items.)

      1. Describe the child's disability (include all behavioral characteristics manifested by the child as a condition of the disability and their severity).

      2. Identify any specific impacts of the disability on the child's functioning (adverse affect on academic achievement, affective and behavioral skills acquisition, and physical/motor development).

      3. Determine if the child has a history of exhibiting this specific misconduct. (If so, describe the frequency of occurrence and severity of similar misbehavior previously noted, and the setting(s) in which it has occurred.)

Text can be captured in two ways:

  1. User entry in the textbox provided alongside the field. The rich-text feature also supports Spell Check ( spell-w16h16.png ) on the inserted text.

  2. Voice-over narration captured as written text by clicking on the Speak Text Icon ( speaktext-w16h16.png ).

  • Select Yes or No, Is there a direct and substantial relationship between the child's disability and the act of misconduct under review?

  • Select Yes or No, Was the conduct in question the direct result of the LEA’s failure to implement the IEP as written?
