The Eligibility Document Summary page displays a description of the document as well as a listing of eligibility documents that the user can select to be completed.
If the student is over age 5, only the School Age Eligibility document listing will appear. If the student is under 5 years old, the Early Childhood Eligibility documents will appear. If the student is age 5, both early childhood and school age sections will be displayed.
The selections made here determine which eligibility sections are added to the document. Only the eligibility documents selected will appear in the flyout and are required to be completed.
Completing the Eligibility Document Summary

Verify student information
Review the student details at the top of the page to ensure accuracy. The student’s name, ID, date of birth, age (as of the creation of the form), Grade and School will be read only and are pulled in from the Student Information profile. -
Select the applicable eligibility categories
Check all disability categories that apply to the student based on evaluations and team input. -
Review added document sections
Once selected, the corresponding eligibility sections will appear in the document navigation menu. -
Complete each eligibility section
Open and fully complete every added eligibility document section.
Once the document is completed, it is imperative that the document is finalized. Finalized documents are not editable.
