The Evaluation Programming Conference/Notice of Decision (EDR) document is applicable only to those student’s with an IEP where a re-evaluation is necessary. When the user attempts to create an EDR document, the student must have the IEP icon appearing next to their name which signifies they have an active IEP.
When an EDR is created for the student, the system will create draft-linked NOA and NOC documents.
Completing the Existing Data Review Document

Select Existing Data Review (EDR) from Create New Document dropdown and click Go.

The document pre-populates the student information:

Student Name

ID

Date of Birth

Age (calculated using the date the document was created)

Grade School/Site

Check the Will Sign Paper Copy checkbox if the document will be signed via paper copy.
Will Sign Paper Copy
Select Will Sign Paper Copy if signatures will be collected on a paper copy and not via electronic signature. If signatures will be collected electronically, refer to “How to sign a document through digital signature”.

The Current Eligibility Category will be auto-populated from the Disability value located on the Special Education Information profile. The section is read-only and cannot be edited.

Section I. Data Reviewed. Select one or more items for Data Reviewed.
Note: File attachments can be linked to the document by clicking on Browse/Drop File Here to upload relevant files (reports, assessments). Attaching files are not required. Read here to know how to upload relevant documents.

When selecting Other Data (specify), enter required text.

Section II. Decisions – Is additional data needed to determine: Select Yes or No for each of the questions.

If at least one of the questions has a Yes response, then user will complete section A. If additional data is needed … the IEP Team recommends … by selecting an option below. Section B will not be editable. Section A is completed, upon finalization of the EDR document, a draft EPC document will be created.

Evaluation/reevaluation to determine eligibility or continued eligibility.

Reevaluation to consider new/additional eligibility.
-
Reevaluation to gather information to aid in instructional planning.

Committee determined evaluation - check at least one of the areas of assessment.

If all responses to the 4 questions are No, then the user will complete section B. If additional data is not needed, the team has determined ….. by selecting an option below. Section A will not be editable. Check one determination:

Child meets IDEA eligibility or, in the case of a reevaluation, continues to meet IDEA eligibility.

If this option is selected, a disability category is displayed and is required. Using the “Determination Date” value on document compared to the student’s DOB, if the student is between 3 years and 5 years, 11 months and grade is NOT Kindergarten, the student is considered early childhood. Otherwise the student is considered school age. When the user attempts to finalize the document, it will confirm there is a school-age or early childhood applicable disability selected.

Upon finalization of the document:

On the Special Education Information profile will be updated as follows:

The Disability field will flow the value from the EDR document.
-
The Eligibility Outcome field will be set as Eligible.

The EDR Determination Date value will flow to the Eligibility Determination Date.

Creates (if a different disability is selected from what the student currently has) a new Disability History record using the Determination Date (bottom of page) as the new disability start date and, if a previous disability record exists, ending the previous assigned disability the date prior to the new start date.

A new Eligibility History profile record is created where the EDR Determination Date is the Eligibility Date and the Eligibility Outcome value is Eligible.

Child does not meet IDEA eligibility or, in the case of a reevaluation, does not continue to meet IDEA eligibility.

If this option is selected, the student no longer is eligible for special education services. Upon finalization of the document, the Disability History record will be ended using the Determination Date as the disability end date.

On the Special Education Information profile will be updated as follows:

The Disability field will no longer have a value (disability is cleared).
-
The Eligibility Outcome field will be set as Not Eligible.
-
The EDR Determination Date value will flow to the Eligibility Determination Date.

Ends the open/current Disability History record using the Determination Date (bottom of page) as the new disability end date.

A new Eligibility History profile record is created where the EDR Determination Date is the Eligibility Date and the Eligibility Outcome value is Not Eligible.

Parent Agreement to Excusal:

If “Will Sign Paper Copy” checkbox is not checked, the user is required to select which parent/guardian will be required to select the Yes/No and initial when signing digitally.

If “Will Sign Paper Copy” checkbox is checked, no selections are required.

Yes/No and Parent Initials are completed via paper copy or via the digital signature process.

User is required to select a response (yes or no) regarding the excused member’s area of curriculum being discussed.

Enter the Date the determination was made. This is a required field. This date value flows to various student profiles. See above for details.

If there are required fields not yet completed, the system will display the following error message:

Office Use Only Section

From the flyout menu, select Office Use Only.

Use the date-picker to select a Date of delivery.
-
Use the drop-down list to select the Mode of delivery.

When the document is finalized, the date and mode of delivery will flow to the student’s Document Delivery profile.
Once the EDR has been completed, it is imperative that the document is finalized so that applicable data flows to the student’s profiles for state and federal reporting as well as any data integration to eSchoolPLUS.
