Document: Evaluation Planning

An additional Prior Written Notice section has been added to the Evaluation Planning document. When the 'Initial evaluation of your child' checkbox is selected, a new 'PWN - Initiate' section would be added to the document. This change aims to accommodate the requirement of filling out two PWNs for all initial referrals, one for initiating the referral and another for initiating the evaluation. The addition of this section is intended to prevent both offerings from being listed on one PWN.

The 'Summary of Services Matrix' section now requires at least one service to be entered in order to complete the section, whereas previously the section could be completed without any data entered.
