ADD "Medical Practitioner Authorization Form

Added the state-mandated Medical Practitioner Authorization Form (MPAF) as a standalone document in Pennsylvania Special Programs, directly below the Medical Assistance Program Billing Consent. The form supports digital signature workflows with staff lookup and role validation, ensuring that only signature and date fields are editable during the signing process.

MA Program Billing Consent inital checkbox

Resolved an issue during Pennsylvania IEP document creation where unchecking the MA Program Billing Consent checkbox still included or triggered the form. When unselected during document setup, the MA Program Billing Consent form is now properly excluded from the IEP package.

EVALUATION REPORT Agree and disagree checkboxes

Updated the Pennsylvania Evaluation Report (ER) signature page logic so the Agree and Disagree checkboxes are required and editable whenever the Determination of Specific Learning Disability (SLD) section has been completed. This applies consistently across all SLD evaluation scenarios without depending on previous Determining Factors field checks.

REEVALUATION REPORT Agree and disagree checkboxes

Updated the Pennsylvania Reevaluation Report (RR) signature page logic so the Agree and Disagree checkboxes are required and editable whenever the Determination of Specific Learning Disability (SLD) section has been completed. This ensures proper signature capture across all reevaluation outcomes, including when a student qualifies, qualifies under another category, or no longer qualifies for special education.
