Notice of Action (NOA) Document

A Notice of Action (NOA) The LEA must obtain informed consent from the parent of the child referred for an initial evaluation. Based on Referral Conference Decision, the committee would identify and determine necessary evaluations for the referred child. The Notice of Action would be completed by the committee and provided to the parent(s) to obtain informed consent for the proposed evaluations or to notify the parent(s) of the decision not to conduct an evaluation. The purpose of providing written notice to the parents is to ensure they understand what action the LEA is proposing or refusing (in this case, to conduct an initial evaluation) and what that decision was based on. The Notice of Action should be in the parent’s preferred language or mode of communication. The parents will sign the Notice of Action to provide written consent for evaluation.

The NOA document will be used to flow compliant data such as consent to test outcome and consent date, special education exit date and applicable reason based on how the document is completed. Please refer to the chart below which outlines the various selections of the NOA and the applicable data flows to profiles.

A guided action automatically creates a Notice of Action document when the following documents are created and saved:

Completing the Notice of Action Document

  • Select the Notice of Action link associated with the appropriate document. The Notice of Action consists of the following sections: 

    • Notice of Action

    • Office Use Only   

  • The document pre-populates the student information from the Student Information profile: 

    • Student Name 

    • ID

    • Date of Birth

    • Age (calculated using the “Date Provided” value)

    • Grade

    • School/Site

  • Parent Name and Address are auto-populated.

NOTE: Parent/guardian names prepopulate from the Student Contacts profile.  Contacts with “Receives Mail” checkbox selected (connected to Registration Labels in eSchoolPLUS) will appear in the document as read only. If data is incorrect or blank, the user must correct it in eSchoolPLUS and refresh the integration process. 

  • Complete the Date Provided (required field).

  • In the Method of Provision, select at least one of the following:

    • In person

    • By Mail

    • E-mail

  • Check the Will Sign Paper Copy box if the document will be signed via paper copy. If the checkbox is not checked, the document will be sent out via Digital Signature.

Select Will Sign Paper Copy if parental response and consent will be collected on a paper copy and not via electronic signature. If the parental response will be collected electronically, refer to “How to sign a document through digital signature”. 

  • Complete “The following is to describe the action(s)”, select the appropriate option (at least one checkbox is required)

    • Proposed

    • Refused by our district

  • Indicate which actions require consent in Box A by selecting one or more checkbox(es), if applicable. 

    • When Initial Placement is checked and the student is age 3 years to 6 years, 11 months as of the date the user created the NOA document, an additional field labeled “IEP Type” will be displayed and is required. The user must indicate if the parent is consenting to an Early Childhood or School Age IEP. Upon finalization of the NOA, the IEP Type will flow to the Referral profile to the field labeled IEP Type. When the student is age 7 or older as of the date the NOA is created, the IEP Type label and drop down will not display. Upon finalization of the document, the system will flow the value of School Age IEP No Post Secondary to the Referral profile.

    • When Reevaluation is selected, then Comprehensive Evaluation, Specialized Evaluation, or Other must be selected.

    • When Other is selected, a required text box appears.

  • Indicate which actions do not require consent in Box B by selecting one or more checkbox(es), if applicable. 

    • When Other is selected, a required text box appears.

  • Select the checkbox Parent agreed to immediate implementation of the action being proposed if the parent/guardian has Consent Granted for the Action to Take Effect Immediately. See below chart as this checkbox will determine the data value to use for consent/exiting etc.

  • Complete the following required long text field entries:

    • Explanation of Action

    • Basis for the Action

    • Options Considered and Reasons Rejected

    • Other Factors Relevant to the Action

  • The ‘Informed Consent’ section is used to capture the name, signature and date information for individuals providing consent. If collecting consent electronically (Will sign paper copy is not checked) then the user is required to select with whom the document will be sent to digitally (parent/student/other):

    • Parents or Guardians

    • Student (displays the option if 18 years of age or older as of the date the document was created)

    • Others

  • The ‘Procedural Safeguards’ section, the first contact name will auto-populated as identified in the Global Profile (Special Education Director) and can be changed. The contact’s title, phone number and email address will also pull into the document if it is populated in the Staff’s profile.

  • Complete “If you have any questions or concerns regarding this action, please contact:” by selecting staff using look-up or select non-look-up and manually enter the Name, Phone Number and Email.

  • Click Save, Done Editing.

When saving the document without completing all the required fields, the system will display the following error message:

State Compliance Data

When a Notice of Action (NOA) is finalized, the application automatically updates student records based on the selected action, parent consent status, and checkbox selections. The system flows consent dates, sets completion and exit reasons, creates required draft documents (EPC, IEP, exit history), and adds or removes REF and IEP tags as needed. For exit-related actions, effective dates are calculated based on parent agreement to immediate implementation, ensuring accurate timelines and compliance without manual updates.

  • The student’s REF tag will be removed when the script process is run.

Box B. Graduation with Regular Diploma

N/A

  • Special Education Information Profile:

    • If the user checks “Parent agreed to immediate implementation of the action being proposed” checkbox, the system will flow the Date Provided value to the Special Education Exit Date field otherwise the system adds 7 calendar days to the Date Provided value.

    • Flows the value “Graduated with Regular Diploma” to the Special Education Exit Reason field.

  • Creates a Special Education Exit History profile record.

  • The student’s IEP tag is removed.

Box B. Dismissal from Special Education Services

N/A

  • Special Education Information Profile:

    • If the user checks “Parent agreed to immediate implementation of the action being proposed” checkbox, the system will flow the Date Provided value to the Special Education Exit Date otherwise the system adds 7 calendar days to the Date Provided value.

    • If the student is in Kindergarten, the Special Education Exit Reason value will flow a value of “Student returned to the regular classroom (RC)”. If the student is not in Kindergarten, the value will be “No longer requires special education services (NS)”.

    • Clears the disability value.

  • The system will end the Disability History profile using the Date Provided date or adds 7 days from the Date Provided value if Parent agreement to immediate implementation of the action being proposed (checkbox is checked).

  • The student’s IEP tag is removed.



Office Use Only Section

This section captures the Delivery status and response status of the Notice of Action document. When the document is finalized, the date and mode of delivery will flow to the student’s Document Delivery profile.

  • Use the drop-down list to select the Mode of delivery.

  • Use the date picker to select a Date of delivery.

  • Consent Response drop down will appear if any of the values in Box A are checked as consent is required for Box A.

  • Consent Date field will display (and is required) when the user selects Granted as the Consent Response.

  • Click Save, Done Editing.

If submitting for digital signature, set the document status to Review. Once the document has been signed (paper or digitally), it is critical that the document is Finalized so that the applicable compliance data flows to the student’s profiles then available for state reporting and data integration to eSchoolPLUS as well as the appropriate special education draft forms.
