A Notice of Action (NOA) The LEA must obtain informed consent from the parent of the child referred for an initial evaluation. Based on Referral Conference Decision, the committee would identify and determine necessary evaluations for the referred child. The Notice of Action would be completed by the committee and provided to the parent(s) to obtain informed consent for the proposed evaluations or to notify the parent(s) of the decision not to conduct an evaluation. The purpose of providing written notice to the parents is to ensure they understand what action the LEA is proposing or refusing (in this case, to conduct an initial evaluation) and what that decision was based on. The Notice of Action should be in the parent’s preferred language or mode of communication. The parents will sign the Notice of Action to provide written consent for evaluation.
The NOA document will be used to flow compliant data such as consent to test outcome and consent date, special education exit date and applicable reason based on how the document is completed. Please refer to the chart below which outlines the various selections of the NOA and the applicable data flows to profiles.
A guided action automatically creates a Notice of Action document when the following documents are created and saved:

Completing the Notice of Action Document

If data is incorrect or blank, the user must correct it in eSchoolPLUS and refresh the integration process.

When Initial Placement is checked and the student is age 3 years to 6 years, 11 months as of the date the user created the NOA document, an additional field labeled “IEP Type” will be displayed and is required. The user must indicate if the parent is consenting to an Early Childhood or School Age IEP. Upon finalization of the NOA, the IEP Type will flow to the Referral profile to the field labeled IEP Type. When the student is age 7 or older as of the date the NOA is created, the IEP Type label and drop down will not display. Upon finalization of the document, the system will flow the value of School Age IEP No Post Secondary to the Referral profile.

The ‘Informed Consent’ section is used to capture the name, signature and date information for individuals providing consent. If collecting consent electronically (Will sign paper copy is not checked) then the user is required to select with whom the document will be sent to digitally (parent/student/other):

The ‘Procedural Safeguards’ section, the first contact name will auto-populated as identified in the Global Profile (Special Education Director) and can be changed. The contact’s title, phone number and email address will also pull into the document if it is populated in the Staff’s profile.

Complete “If you have any questions or concerns regarding this action, please contact:” by selecting staff using look-up or select non-look-up and manually enter the Name, Phone Number and Email.

Click Save, Done Editing.
When saving the document without completing all the required fields, the system will display the following error message:

State Compliance Data
When a Notice of Action (NOA) is finalized, the application automatically updates student records based on the selected action, parent consent status, and checkbox selections. The system flows consent dates, sets completion and exit reasons, creates required draft documents (EPC, IEP, exit history), and adds or removes REF and IEP tags as needed. For exit-related actions, effective dates are calculated based on parent agreement to immediate implementation, ensuring accurate timelines and compliance without manual updates.

This section captures the Delivery status and response status of the Notice of Action document. When the document is finalized, the date and mode of delivery will flow to the student’s Document Delivery profile.

Consent Response drop down will appear if any of the values in Box A are checked as consent is required for Box A.

Consent Date field will display (and is required) when the user selects Granted as the Consent Response.

If submitting for digital signature, set the document status to Review. Once the document has been signed (paper or digitally), it is critical that the document is Finalized so that the applicable compliance data flows to the student’s profiles then available for state reporting and data integration to eSchoolPLUS as well as the appropriate special education draft forms.
