PWN Exit from Initial Evaluation

Creating the Prior Written Notice Document to Exit from Initial Evaluation

  1. Select a student on the Students panel.

  2. Click the Documents icon for the student.

  3. From the Create New Document drop-down list, select Prior Written Notice – Exit from Special Education, and click Go.

  4. Creation Constraints: The document can only be created if the student is in the referral process (REF icon appears next to the students name) or the date of referral is greater than 41 days from “todays date”.

  5. In the Student Information section, the system auto-populates the following information:

    • From the Student Info/Demo Part B profile:

      • Student

      • USI

      • DOB

    • From the LEA Entry/Exit Part B profile:

      • Grade

    • From the Special Education Part B profile:

      • Case Manager

  6. Enter a date manually or select a date using the calendar icon.

  7. Select the name(s) of Parents/Guardians.

  8. Student Name and full address, including Street Address, City, State, and Zip Code, are auto-populated.

  9. Enter a detailed long-text response for the following three questions.

    1. Description of the proposed or refused action:

    2. Explanation of why the LEA is proposing or refusing the action:

  10. For the question: The initial evaluation process cannot proceed due to, choose the appropriate response from the given options.

    • The parent or adult student has not responded to requests for consent for the initial evaluation process

    • The parent or adult student has not responded to requests to make the student available for evaluation activities

    • Other

  11. Enter a detailed long-text response for the following three questions.

    1. Description of each evaluation procedure, assessment, record, or report used as a basis for the proposed or refused action.

    2. Description of other options considered by the IEP Team and reason(s) for rejecting those options.

    3. Description of other factors related to the proposal or refusal.

  12. Finalize the document upon completion. To finalize the document, click Set Document, select Status from Draft to Final, and then click Accept.

  • If the Red Flag option is selected in the student Profile, Student Info/Demo Part B, then the following information for a student or parent does not appear when the document is finalized:

    • Address

    • Phone number or Contact information

  • If more than one parent has Educational Rights selected on the student's Profile, Student Contacts Part B, the system provides a list of the parent or guardian names with a checkbox. The user must select the name of the parent or guardian who will sign digitally.

  • Click More, Update Document from Student Profile to refresh parent or guardian details on the document when the document is in draft mode. Parent or guardian details cannot be updated for documents in Review or Final mode.

Document Delivery

This section is required when the user selects Parent/Adult Student will sign a paper copy checkbox. The section does not print.

  1. Navigate to Document Delivery using the flyout menu.

  2. Select the Mode of Delivery from the drop-down list.

  3. Select the Date of Delivery.

  4. Click Save, Done Editing.

Data Flow

Upon finalization of the documents, the following data flow occurs:

To Profile

Flows the date of delivery value to the Referral profile, Referral Discontinued Date field.

The REF icon will then be removed on the next refreshing process as there is a discontinued date value.

Referrals Part B

Creates a new PWN Part B profile record using the date of delivery in the Document Delivery section as the PWN Date.

Proposal value is Proposing. Action value is Exit. Event value is Initial Evaluation.

PWN Part B

Creates a Document Delivery profile record using the date of the document as the Document Date value.
