NOTE: When the IEP is amended, the cover page is read-only. A new Amendment cover page will appear in the amendment.

Completing the IEP Cover Page

Select IEP document and add an optional Label/Comment and click New. The system will create a draft Individualized Education Program document.

The system will open to the cover page. The Student Name, ID, Date of Birth, English Learner, Grade and School will pull from the Student Information Profile. The student’s Disability will pull in from the Special Education Information profile and will flow to the newly created Plan History record.

If signatures will be collected with a paper copy, check Will Sign Paper Copy.
Will Sign Paper Copy
Select Will Sign Paper Copy if signatures will be collected on a paper copy and not via electronic signature.

Enter the Duration of Services From and To dates (required).

Under Purpose of IEP, select the applicable purpose checkbox and enter the meeting date. Once the meeting date is populated, the system will refresh the screen and will select the IEP Type value. When the IEP meeting purpose is Initial,

IEP Type will display based upon the student’s age as of the meeting date.

If the student is age 6 or less, the user can select either Early Childhood IEP or School Age IEP.

If the student is age 6 and less than age 16, the text will display as School Age - No Postsecondary Transition. If the user manually adds the Post-Secondary Transition section, the IEP Type will change to School Age Post Secondary Transition IEP.

When the student is 16 or older as of the meeting date, the IEP type displays as School Age Post Secondary Transition IEP.

Once the IEP Type value is indicated, the document will insert or remove the applicable sections.

The Most Recent Evaluation Date will flow from the Special Education Information profile, Most Recent Evaluation Date value.

Select from one of the required checkboxes Parent Rights Provided: Personally Presented, Mail, Email [at least one option must be selected, user may select more than one]

Indicate whether Translation/Interpretation Needed: if select Yes, then a required text box displays. If select No, no additional entry required.

Select from of the required checkboxes Parent Method of Participation: In Person, Through alternate means, Parent did not participant [user must select at least one option]

The Parent Agreement to Excusal

If signing digitally (Will Sign Paper Copy checkbox is not checked), the user must select a parent/guardian so that the system will require the person to select yes/no and initial during the digital signature process. If Will Sign Paper Copy checkbox is checked, there is no prompt to select a parent/guardian as the person will sign the paper copy.

Complete Parent/guardian input regarding enhancing the education of the child was considered. Enter a required response.

Select Save, Done Editing.
If there are required fields not yet completed, the system will display the following error message:
Section was saved, but is incomplete until all required fields (pink highlighted) are filled.
Based on the IEP Type, the applicable footer will be displayed.
