Postsecondary Transition planning is a required component of the IEP and focuses on preparing the student for life after high school. Transition goals and services must be in place no later than the first IEP in effect when the student turns 16 years of age, or earlier if determined appropriate by the IEP Team, and must be reviewed and updated annually.
The student must be invited to participate in any IEP meeting where transition planning is discussed. Transition planning is a collaborative process and should reflect the student’s preferences, interests, strengths, and needs.
Postsecondary Transition Services are designed to support the student in working toward identified postsecondary goals in areas such as education or training, employment, and, when appropriate, independent living. The IEP must include a statement describing the student’s strengths and needs, based on age-appropriate transition assessments, as they relate to transition planning. This information reflects the student’s current level of performance and explains how the student’s disability impacts the ability to achieve the postsecondary goals identified in the transition plan.
The section is automatically inserted when the IEP Type on the Cover Page is School Page Postsecondary Transition IEP when the student is at least 15 as of the meeting date on the Cover Page.
If the student is under 15 as of the meeting date, the user can manually add the Post Secondary section manually by clicking Set Document>Sections.
