This profile is essential in tracking a student’s special education journey, ensuring continuity of services, and compliance with IDEA (Individuals with Disabilities Education Act).

In the Enrollment Tracking History Profile of an IEP (Special Education), the terms Responsible School and Attending School refer to different roles within a student’s educational placement and services.

  • The responsible school ensures that the IEP is followed and provides the necessary funding.

  • The attending school is where the student physically receives education and services.

  • This is typically the district where the student's legal residence is located.

    • In a specific case, the student might be placed in a different school for Special services.

  • The responsible school is accountable for funding, IEP development, and ensuring compliance with special education laws.

Enrollment Status

  • The student is concurrently enrolled.

  • The student is currently enrolled.

  • The student is transferring (will enroll).

  • The student was previously enrolled.

Enrollment Start Date

The date on which the Enrollment started.

The date on which the Enrollment ended.

Enrollment Exit Reason

You can pick from one of the following reasons to exit the enrollment:

  • Armed Services Enlistment

  • Court Ordered to Dropout

  • Deceased

  • Disinterest in Curriculum

  • Earned High School Equivalency

  • End of School Year

  • Expulsion, Did Not Return

  • Failure of Graduation Qualifying Exam (GQE)

  • Foreign exchange student who returns to country of origin

  • Graduate

  • Incarcerated or placed in protective custody

  • Interpersonal Problems

  • Missing but Located

  • Missing, students who cannot be located

  • Need to Earn Money

  • Non-Compulsory Attendance Age

  • Participation in the HSE Pilot Program

  • Poor Health

  • Record of School Failure

  • Religious Beliefs

  • Removed by parents to provide Home-School instruction

  • Serving Expulsion

  • Special education student becomes ineligible due to age

  • Transferred

  • Transferred Out of State

  • Transferred to a non-state-accredited non-public school

  • Truancy (Underage No Shows)

  • Unknown or No Shows (Age 18+)
