Some of the fields, as explained in the following table, are pre-populated from the Student Profile and cannot be edited. The remaining fields are mandatory and must be entered manually.

Student Review

Review the present levels of academic achievement and functional performance of the child.

You must log a detailed review and collate answers to the following questions.

  1. Child's strengths and needs (include the data used to determine them)?

  2. Consider the academic, developmental, and functional needs resulting from the child's disability, which may require special education, related services, supplementary aids, supports for personnel, or modifications?

  3. Effect of the child's disability on his/her involvement and progress in the general education curriculum?

  4. Achievement of annual goals, performance in related service areas, and a description of any significant lack of progress?

The response to the above four long text sections can be captured in either of the two ways:
